Dear Mr. Kanipe:
In accordance with your permit modification request dated November 15, 2012 to'add two new
fields to the Town of Columbus' Land Application Program (WQ0002645), we are forwarding
herewith a modified Attachment B, certified January 31, 201.3 to the Town of Columbus for the
subject residuals land application program.
This certification is being issued to modify Attachment B by adding Fields CI -6 (9.51 applicable
acres) and CI -7 (3.9 applicable acres) to the program. These two fields collectively represent
13.41 acres, and their addition to the program increases the total active field acreage from 135.3
acres to 148.71 acres.
Please replace the existing Attachment B of Permit No. WQ0002645 that was previously
certified on November 14, 2008, with this modified Attachment .B. Please also attach
enclosed Figures 4 and 5 to the permit. It is the Pennittee's responsibility to ensure that a
complete permit is created by inserting tbis modified Attachment B and Figures 4 and 5 into
Permit No. WQ0002645, issued on November 14, 2008, and making sure to retain any vicinity
and buffer maps for any existing and unmodified land application sites.
This modified Attachment'B shall be effective from the date of certification; shall void
Attachment B of Permit No.I WQ0002645 that was previously certified on November 14, 2008;
and shall -be- subject to the conditions and limitations as specified therein and well as those
specified in the above -referenced permit. As always, remember to take the time to review this
.modified Attachment B thoroughly, as some of the conditions contained therein may have been
changed since the last certification. Note that this modified Attachment B and Figures 4 and 5 are
hereby incorporated by reference into Permit No. WQ0002645, issued on November 14, 2008,

On August 5, 2009, Session Law 2009-406, entitled "An Act to Extend Certain Government
Approvals Affecting the Development of Real Property Within the State," was enacted by the
General Assembly and signed into law. The Act, known as the Permit Extension Act of 2009,
extends the expiration date of certain government approvals and permits. On August 2, 2010, this
Act was extended an additional year as Session Law 2010-177. The subject permit falls within
the scope of this Act and is therefore being extended to October 31, 2017.
If any parts, requirements, and/or limitations contained in this certification or modified
Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of the certification. This request shall be in the
form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes
and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North
Carolina 27699-6714. Unless such demands are made, this permit shall be final and binding.
