In accordance with your permit modification email request received April 23, 2013; we are
forwarding herewith a modified Attachment B, certified May 23, 2013, to the Town of Angier for the
subject residuals land application program.
This certification is being issued to modify the Attachment B certification to combine:
• Fields 03a and 03b into one Field 03 with acreage of 8.1 and a maximum yearly
application rate of 52.0 inches.
Fields 04a, 04b, and 04c into one Field 04 with acreage of 8.72 and a maximum yearly
application rate of 52.0 inches.
Fields 05a, 05b, 05c, 05d, 05e and 05f into one Field 05 with acreage of 13.68 and a
maximum yearly application rate of 52.0 inches.
Please replace the existing Attachment B of Permit No. WQ0002638 that was previously
certified on April 4, 2008, with this modified Attachment B. It is the Permittee's responsibility to
ensure that a complete permit is created by inserting this modified Attachment B Permit No.
WQ0002638, issued on January 3, 2008, and making sure to retain any vicinity and buffer maps for any
existing and unmodified land application sites.
This modified Attachment B shall be effective from the date of certification; shall void
Attachment B of Permit No. WQ0002638 that was previously certified on April 4, 2008; and shall be
subject to the conditions and limitations as specified therein and well as those specified in the above -
referenced permit. As always, remember to take the time to review this modified Attachment B
thoroughly, as some of the conditions contained therein may have been changed since the last
certification. Note that this modified Attachment B is hereby incorporated by reference into Permit No.
WQ0002638, issued on April 4, 2008.

Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.
