TRACy FORE, WASTEWATER TREATMENT PLANT ORC TOWN OF CI-iADBOURN 208 EAST FIRST AvENuE CHADBOURN, NC 28431 Subject: Attachment A Certification Permit No. WQ0001934 Town of Chadbourn Town of Chadboum Residuals Land Application Program Land Application of Residual Solids (503) Columbus County Dear Mr. Fore: In accordance with your permit modification application package received on March 19, 2003, we are forwarding herewith a modified Attachment A, certified August 29, 2003, to the Town of Chadbourn for the subject residuals land application program. This certification is being issued to modify Attachment A to reduce the frequency of monitoring the residuals for the hazardous waste characteristics of TCLP, Corrosivity, Ignitability, and Reactivity from annually to once each permit cycle (every 5 years). Although this monitoring can be conducted at any time during the permit cycle, it is recommended that the monitoring for hazardous waste characteristics be conducted in anticipation of preparing a permit renewal application package as it must be submitted with that package and this will ensure that the monitoring is current at the time of renewal. As always, remember to take the time to review this mortified Attachment A thoroughly, as some of the conditions contained therein may have been changed since the last certification. Of particular interest to you may be the following: Information presented in the permit modification application package states that the town has no major industries discharging into the sewer system. Therefore, this residuals source -generating facility has been approved to monitor for the hazardous waste characteristics required by Condition III, 2. of the permit at the frequency of once per permit cycle (every 5 years). The once per permit cycle monitoring frequency for Condition III.2. is acceptable only as long as the wastewater being treated by the facility is domestic in nature. As noted on footnote a to Attachment A, if any wastewater is introduced to this facility for treatment from a Significant Industrial User (SILT) as defined by15 NCAC 02H .0903(b)(34), such monitoring shall be required on at least an annual frequency. Please replace the existing Attachment A of Permit No. WQ0001934 that was previously certified on March 15, 2002, with this modified Attachment A. It is the Permittee's responsibility to ensure that a complete permit is created by inserting this modified Attachment A into Permit No. WQ0001934, issued on March 15, 2002. Note that this modified Attachment A is hereby incorporated by reference into Permit No. WQ0001934, issued on March 15, 2002. If any parts, requirements, and/or limitations contained in this certification or modified Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the from of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such demands are made, this permit shall be final and binding.
