Dear Mr. Kairis:
On October 3, 2005, the Division of Water Quality (Division) received a correspondence from
Ms. Anne Hice of the Kearns Family Limited Partnership regarding the above -referenced permit. Ms.
Hice's letter provided notification that she wishes to remove land owned by Iand owned by Ms. Mary D.
Kearns and operated by Mr. Gene Hiatt and Mr. Bob McClintock from the list of approved land
application sites that may receive residuals under the conditions of this permit. Ms. Hice has power of
attorney for Ms. Kearns, has provided a similar notice to you under separate cover, and intends to sign a
landowner agreement with the City of Randleman to- transfer the affected sites into its residuals land
application program (i.e., Permit No. WQ0004825) in the near future. Therefore, we are forwarding
herewith an administratively -amended Attachment B of Permit No. WQ0401897, certified on December
6, 2005, to the City of High Point for the subject residuals land application program.
This Attachment B is being administratively amended to remove Site Nos. NC-GU-085-01, NC-
GU-085-02, NC-GU-085-03, and NC-GU-085-04 from the list of land application sites that are approved
to receive residuals under this residuals land application program. All of the sites are owned by Ms.
Kearns and operated by Mr. Hiatt and Mr. McClintock, are located in Guilford County, contain 42.00
acres of application area, and are stated by Ms. Hice as being planned for inclusion in a permit
modification application to be submitted to the Division by the City of Randleman for approval under its
residuals land application program (i.e., Permit No. WQ0004825) soon.
Please replace the existing Attachment B of Permit No. WQ0001897 that was previously
certified on January 31, 2003 and corrected on April 14, 2003, with this administratively -amended
Attachment B. This administratively -amended Attachment B shall be effective from the date of
certification; shall void Attachment B of Permit No. WQ0001897 that was previously certified on January
31, 2003 and corrected on. April 14, 2003; and shall be subject to the conditions and limitations as
specified therein as well as those specified in the above -referenced permit. As always, remember to take
the time to review this administratively -amended Attachment B thoroughly, as some of the conditions
contained therein may have been changed since the last certification. Note that this administratively -
amended Attachment B is hereby incorporated by reference into Permit No. WQ0001897, issued on

If any parts, requirements, and/or limitations contained in this certification or administratively -
amended Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon
written request within 60 days following receipt of the certification. This request shall be in the form of a
written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the
Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
Unless such demands are made, this Attachment B shall be final and binding.

6 No residuals shall be applied to this land application site until all tree stumps are removed and a vegetative cover is established.
` Reduced buffers from residences under separate ownership have been approved for this land application site. Residuals may be land applied to within 100 feet ofresidences, regardless
of whether a surface or sub -surface application method is utilized. Note that no reduction in the required buffer between the land application area and property lines or public right-of-
way is allowed. A revised buffer waiver form from the new owner shall be obtained in case of a change in ownership of these residences.
