This permit shall be effective from the date of issuance until May 31, 2004, shall void Permit No. WQ0001814 issued September 30, 1994, and shall be subject to the conditions and limitations as specified therein. Please pay particular attention to the monitoring requirements in this permit. Failure to establish an adequate system for collecting and maintaining the required operational information will result in future compliance problems. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, P.O. Stevens cc: Nash County HD RRO, WQS RRO, GWS Bob Cheek, GWS, CO Technical Assistance and Certification Unit Non -Discharge Compliance/Enforcement Unit at (919) 733-5083 PO Box 29535, Raleigh, North Carolina 27626-0535 Telephone (919) 733-50B3 Fax (919) 733-0719 An Equal Opportunity Affirmative Action Employer 50% recycled110% post -consumer paper NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF ENVIRONMENT AND NATURAL RESOURCES RALEIGH SPRAY IRRIGATION SYSTEM PERMIT In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO International Paper Masonite Division Nash County FOR THE continued operation of a 5,000 GPD extended aeration type wastewater treatment plant with chlorination, for treating domestic wastes which feeds into a 216,000 GPD industrial and domestic wastewater spray irrigation treatment and disposal facility consisting of an influent pump station, a hydrosieve for solids removal, chemical (lime) treatment tanks, a polymer feed system, a primary clarifier, dual 600,000 gallon aeration basins with mechanical aeration, two (2) sludge holding ponds, flow measuring and recording equipment, a long term (50 MG) holding pond and two (2) spray irrigation fields; one consisting of approximately 16 acres and the other approximately 5 acres to serve Intemational Paper's Masonite Division (approximately 10,972 GPD spray irrigated), with no discharge of wastes to the surface waters, pursuant to the application received April 15, 1999, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Environment and Natural Resources and considered a part of this permit. This permit shall be effective from the date of issuance until May 31, 2004, shall void Permit No. WQ0001814 issued September 30, 1994, and shall be subject to the following specified conditions and limitations: I. PERFORMANCE STANDARDS The spray irrigation facilities shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters which will render them unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions or failure of the irrigation area to adequately assimilate the wastewater, the Permittee shall take immediate corrective actions including those actions that may be required by the Division of Water Quality (Division), such as the construction of additional or replacement wastewater treatment and disposal facilities. 2. The issuance of this permit shall not relieve the Permit -tee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. The residuals generated from these treatment facilities must be disposed in accordance with General Statute 143-215.1 and in a manner approved by the Division. 4. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. The following buffers shall be maintained: a) 400 feet between wetted area and any residence or places of public assembly under separate ownership, b) 150 feet between wetted area and property lines, c) 100 feet between wetted area and wells, d) 100 feet between wetted area and drainageways or surface water bodies, e) 50 feet between wetted area and public right of ways, f) 100 feet between wastewater treatment units and wells, and g) 50 feet between wastewater treatment units and property lines. Some of the buffers specified above may not have been included in previous permits for this waste treatment and disposal system. These buffers are not intended to prohibit or prevent modifications, which are required by the Division, to improve performance of the existing treatment facility. These buffers do, however, apply to modifications of the treatment and disposal facilities which are for the purpose of increasing the flow that is tributary to the facility. These buffers do apply to any expansion or modification of the spray irrigation areas and apply in instances in which the sale of property would cause any of the buffers now complied with, for the treatment and disposal facilities, to be violated. The applicant is advised that any modifications to the existing facilities will require a permit modification. If, at any time, this facility closes with no prospect of becoming operational again, then the Raleigh Regional Office, (919) 571-4700, must be contacted for procedures to follow for closure of the facility. II. OPERATION AND MAINTENANCE REQUIREMENTS The facilities shall be properly maintained and operated at all times. 2. Upon classification of the facility by the Water Pollution Control System Operators Certification Commission (WPCSOCC), the Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment facilities by the WPCSOCC. The Permittee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of 15A NCAC 8A .0202. The ORC of the facility must visit each Class I facility at least weekly and each Class I1, III, and IV facility at least daily, excluding weekends and holidays, and must properly manage and document daily operation and maintenance of the facility and must comply with all other conditions of 15A NCAC 8A .0202 3. A suitable year round vegetative cover shall be maintained. 4. Irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff. Adequate measures shall be taken to prevent wastewater runoff from the spray field. 2 6. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 7. The application rate shall not exceed a cumulative loading of 41 inches over any twelve (12) month period at an instantaneous application rate not to exceed 0.20 inches per hour. 8. No type of wastewater other than that from International Paper — Masonite Division shall be sprayed onto the irrigation area. 9. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal maintenance is being performed. 10. Public access to the land application sites shall be controlled during active site use_ Such controls may include the posting of signs showing the activities being conducted at each site. 11. Freeboard in the 50 MG holding pond shall not be less than two feet at any time. 12. The domestic waste effluent must be chlorinated before being added to the industrial wastewater. III. MONITORING AND REPORTING REQUIREMENTS Any monitoring (including groundwater, surface water, soil or plant tissue analyses) deemed necessary by the Division to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 2. Adequate records shall be maintained by the Permittee tracking the amount of wastewater disposed_ These records shall include, but are not necessarily limited to, the following information: a. date of irrigation, b. volume of wastewater irrigated, c. field irrigated, d. length of time field is irrigated, e. continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings for each field, f. weather conditions, and g. maintenance of cover crops. 3. The effluent from the subject facilities shall be monitored by the Permittee at the point prior to irrigation every February, June and October for the following parameters: BOD5 TSS Fecal Coliform pH NH3 as N Sodium Magnesium Calcium Sodium Adsorption Ratio by Calculation 4. Three (3) copies of all operation and disposal records (as specified in condition III 2) on Form NDAR-1 shall be submitted on or before the last day of the following month. Three (3) copies of all effluent monitoring data (as specified in condition III 3) on Form NDMR-1 shall be submitted on or before the last day of March, July and November. Noncompliance Notification: The Permittee shall report by telephone to the Raleigh Regional Office, telephone number (919) 571-4700, as soon as possible, but in no case more than 24 hours or on the next working day following the occurrence or first knowledge of the occurrence of any of the following: a. Any occurrence at the wastewater treatment facility which results in the treatment of significant amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the contents of a sludge digester; the known passage of a slug of hazardous substance through the facility; or any other unusual circumstances. b. Any process unit failure, due to known or unknown reasons, that render the facility incapable of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators, compressors, etc. c. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass directly to receiving waters without treatment of all or any portion of the influent to such station or facility. d. Any time that self -monitoring information indicates that the facility has gone out of compliance with its permit limitations. Persons reporting such occurrences by telephone shall also file a written report in letter form within five (5) days following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not recur. GROUNDWATER REQUIREMENTS Monitoring wells MW-I, MW-2, MW-3 and MW-4 shall be sampled initially after construction and thereafter every February, June and October for the following parameters: Water Level Chlorides Total Organic Carbon (TOC) pH Total Dissolved Solids (TDS) Total Ammonia Volatile Organic Compounds - In February only using one of the following: (A) Standard Method 6230D, PQL at 0.5 ug/L or less (B) Standard Method 621 OD, PQL at 0.5 ug/L, or less (C) EPA Method 8021, Low Concentration, PQL at 0.5 ug/L or less (D) EPA Method 8260, Low Concentration, PQL at 0.5 ug/L or less (E) Another method with prior approval by the Groundwater Section Chief Any method used must meet the following qualifications: (1) A laboratory must be DWQ certified to run any method used. (2) The method used must, at a minimum, include all the constituents listed in Table VIII of Standard Method 6230D. 4 (3) The method used must provided a PQL at 0.5 ug/L or less which must be supported by laboratory proficiency studies as required by the DWQ Laboratory Certification Unit. Any constituents detected above the MDL but below the PQL at 0.5 ug/L must be qualified (estimated) and reported. If any volatile organic compounds are detected by the methods listed, then the Raleigh Regional Office Groundwater Supervisor, telephone number (919) 57I-4700, must be contacted immediately for further instructions regarding any additional follow-up analyses required. The results of all initial and follow-up analyses must be submitted simultaneously. The measurement of water levels must be made prior to sampling for the remaining parameters. The depth to water in each well shall be measured from the surveyed point on the top of the casing. If TOC concentrations greater than 10 mg/I are detected in any downgradient monitoring well, additional sampling and analysis must be conducted to identify the individual constituents comprising this TOC concentration. If the TOC concentration as measured in the background monitor well exceeds 10 mg/l, this concentration will be taken to represent the naturally occurring TOC concentration. Any exceedances of this naturally occurring TOC concentration in the downgradient wells shall be subject to the additional sampling and analysis as described above. The results of the sampling and analysis must be received on Form GW-59 (Groundwater Quality Monitoring: Compliance Report Form) by the Groundwater Section, Permits and Compliance Unit, P.O. Box 29578 Raleigh, N.C. 27626-0578 on or before the last working day of the month following the sampling month. 2. Any additional groundwater quality monitoring, as deemed necessary by the Division, shall be provided. The COMPLIANCE BOUNDARY for the disposal system is specified by regulations in 15A NCAC 2L, Groundwater Classifications and Standards. The Compliance Boundary is for the disposal system constructed after December 31, 1983 is established at either (1) 250 feet from the waste disposal area, or (2) 50 feet within the property boundary, whichever is closest to the waste disposal area. An exceedance of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to immediate remediation action in addition to the penalty provisions applicable under General Statute 143-215.6A(a)(1). In accordance with 15A NCAC 2L, a REVIEW BOUNDARY is established around the disposal systems midway between the Compliance Boundary and the perimeter of the waste disposal area. Any exceedance of standards at the Review Boundary shall require remediation action on the part of the permittee. V. INSPECTIONS Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of wastes to the environment, a threat to human health, or a nuisance. The Permittee shall keep an inspection log or summary including at Ieast the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall be maintained by the Permittee for a period of three years from the date of the inspection and shall be made available upon request to the Division or other permitting authority. 3. Any duly authorized officer, employee, or representative of the Division may, upon presentation of credentials, enter and inspect any property, premises or place on or related to the disposal site or facility at any reasonable time for the purpose of determining compliance with this permit; may inspect or copy any records that must be maintained under the terms and conditions of this permit, and may obtain samples of groundwater, surface water, or leachate. VI. GENERAL CONDITIONS This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. 4. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to an enforcement action by the Division in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. 6. A set of approved plans and specifications for the subject project must be retained by the Penn ittee for the life of the project. 7. The annual administering and compliance fee must be paid by the Permittee within thirty (30) days after being billed by the Division. Failure to pay the fee accordingly may cause the Division to initiate action to revoke this permit as specified by 15A NCAC 2H .0205 (c)(4). The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the permit for such period of time and under such conditions and limitations as it may deem appropriate. AOD4 DERN71ONS The following definitions shall apply throughout this Subchapter. (1) Activated Carbon Beds. A physlcalichemical method for reducing soluble organic material from wastewater effluent; The column -Type beds used In this method will have a flow rate varying from two to eight gallons per minute per square foot and may be either vpflow or downflow carbon beds. Carbon may or may not be regenerated on the wastewater treatment plant rile; (2) Aerated Lagoons. A basin in which all solids are maintained In suspension and by which biological oxidation or organic matter Is reduced through artificilOy accelerated transfer of oxygen on a Clow -through basis; (3) Aeration. A process of bringing about intimate contact between air or high purity oxygen In a liquid by spraying, agitation or diffuslon;(3a) Extended Aeration. An activated sludge process utilizing a minimum hydraulic detention time of 18 hours (4) Agricufturally managed site. Any she on which a crop Is produced, managed, and harvested (Crop includes grasses, grains, trees, etc.); (5) Air Stripping. A process by which the ammonium Ion is first convened to dissolved ammonla (pH adjustment) with the ammon)a then released to the atmosphere by physical means; or other similar processes which remove petroleum products such as benzene, toluene, and xylene; (6) Carbon Regeneration The regeneration of exhausted carbon by the use of a furnace to provide extremely high temperatures which volatilize and oxidize the absorbed Impuritlas; (7) Carbonaceous Stage. A stage of wastewater treatment designed 10 achieve 'secondary' effluent limits; (8) Centrifuge. A mechanical aevice In which centrifugal force Is used to separate solids from Ilqulds or to separate liquids of different dsnshlas; (9) Chemical Addition Systems- The add€tlon of chemicals) to wastewater at an application point for purposes of improving solids removal, pH adjustment, alkalinity control, etc.; the capability to expenmern with dMarant chemicals and different application points to achieve a specific result will be considered one system; the capabiitry to add chemical(s) to dual units will be rated as one system; capability to add a chemical at a different application points for different purposes will result in the systems being rated as separate systems; (10) Chemical Sludge Conditioning. The addition of a chemical compound such as Ilme, ferric chloride, or a polymer to wet sludge to coalesce the mass prior to its application to a dewatering device; (11) Closed Cycle Systemac. Use of holding ponds or holding tanks for containment of wastewater containing Inorganic, non -toxic materials from sand, gravel, crushed stone or other similar operations. Such systems shall carry a maximum of two points regardless of pumping faclWas or any other appurtenances; (12) Combined Removal of Carbonaceous BOO and Nitrogenous Removal by Wrtfication- A single stage system required to achieve permit effluent limits on SOD and ammonia nitrogen within the same biological reactor (13) Dechlorinatlon. The partial or complete reduction of residual chlorine in a liquid by any chemical or physical process; (14) Denitr€licatlon Process. The conversion of nitrate-ntrogen to nitrogen gas; (15) Electrodialysis. Process for removing ionized salts from water through the use of ion -selective Ion -exchange membranes; (16) fitter Press. A process operated mechanically for partially dewatering sludge; (17) Foam Saparation. The planned frothing of wastewater or wastewater effluent as a means of removing excessive amounts of delergent materials through the introduction of air in the form of fine bubblcs; also called foam fractionation; (18) Grit Removal_ The process of removing grit and other heavy mineral matter from wastewater; (19) Imhoff Tank. A deep Iwo story wastewater tank consisting of an upper sedimentation chamber and a lower sludge digestion chamber. (20) Instrumented Flow Measurement. A device which Indicates and records rate of flow; (21) Ion Exchange. A chemical process in which ions from two diflerem molecules are exchanged; (22) Land application: (a) Sludge Disposal. A final sludge disposal method by which wet sludge may be applied to land either by spraying on the surface or by subsurface injection chisel plow); tnol applicable for types of sludge described in (11) of this Rule); (b) Treated Effluent. The biochemical conversion of unoxidized nitrogen (ammonia and organic nitrogen) to oxidized nitrogen (usually nitrate); (25) Nitrogenous Stage. A separate stage of wastewater troatmom designed for the specific purpose of converting ammonia nitrogen to nitrate nitrogen; (26) Phosphate Removal, Biological. The removal of phosphorus from wastewater by an oxic/anoxic process designed to enhance luxury uptake of phosphorus by the microorganisms; (27) Pollshing pond. A holding pond following secondary treatment wish sufficient detentlon time to allow senling of finely suspended solids; (28) Post Ae,-ation. Aeration following conventional secondary treatment units to increase effluent Q.O. or for any other purpose; (29) Post Aeration. (Cascade) A polishing method by which dissolved oxygen is added to the effluent by a nonmachanical, gravity means of flowing down a series of steps or weirs; The flow occurring across the steps or weirs moves In a fairly thin layer and the operation of the cascade requires no operalur adjustment; thus, zero points are assigned even though this Is an essential step to meeting the limits of the discharge permit; (30) Powdered to Granular Activated Carbon Feed. A blophysicaf carbon process that utiiizes biological activity and organic absorption by using powdered or granular activated carbon; Virgin or regenerated carbon is lead controlled into the system; (31) Preaeral€on. A tank constructed to provide aeration prior to primary treatment; (32) Preliminary Units. Unit operations in the treatment process, such as screening and comminution, that prepare the liquor for subsequent major operations; (33) Industrial Pretreatment. (a) Pre-treatment Unit, Industrial, The conditioning of a waste at its source before discharge, to remove or to neutralize substances Injurious to sewers and trealmenl processes or to effect a partial reduction in load on She treatment process which is operated by the same governing body as the wastewater treatment plant being rated; b) Pre-treatment Program, Industrial - must be a State or EPA required program to receive points on the rating sheet; (34) Primary Clarifiers. The first sensing tanks through which wastewater is passed in a treatment works for the purpose of removing settleable and suspended solids and BO❑ which Is associated with the solids; (35) Pumps. All inffuent, effluent and in -plant pumps; (36) Radiation. Disinfection or sterilization process utilizing devices emitting ultraviolet or gamma rays; (37) Reverse Osmosis, A treatment process In which a heavy contaminated liquid Is pressurized through a membrane forming nearly pure liquid free from suspended solids; (38) Rotating Biological Contractors. A fixed biological growth process in which wastewater flows through tanks in which a series of partially submerged circular surfaces are rotated; (39) Sand Filters: (a) intenmittant Biological. FlAration of effluent following septic tanks, lagoons, or some olhor treatment process In which further biodecomposltlon is expected to produce desired effluents; Hydraulic loading rates on these fillers are computed In gpd/ac and have a resulting low gpm✓sf (lass than one); b) Recirculating biological . the same type of sand fitter as defined in Subparagraph (39) (a) of this Rule with the added capability to recycle effluent back through the sand filter; (4C) Sand or Mixed -Media Filters. A polishing process by which effluent limits are achieved through a further reduction of suspended solids; (a) low rate -- gravity, hydraullcally loaded filter with loading rates in the one to three gprrt/sf range; (b) high rate -- a pressure, hydraulicaily loaded fitter with loading rates in the five gprwsl range; At any rate, the loading rate will exceed three gprwsf; (41) Secondary Clarifiers. A lank which follows the biological unit of treatment plant and which has the purpose of removing sludges associated with the biological treatment units; (42) Separate Sludge Reaeration. A part of the contact stabilization process where the activated sludge is transferred to a tank and aerated before returning lt to the contact basin; (43) Septic Tank. The process by which organic or volatile matter and sludge is gasified, liquefied, mineralized or corrverled into more stable organic matter through the activity of living organisms, which Includes aerated holding tanks; (45) Sludge Drying Beds. An area comprising natural or artificial layers of porous materials upon which digested sewage sludge is dried by drainage and evaporation; (46) Sludge Elutriatlon, A process of sludge conditioning in which cerlain constituents are removed by successive washings with fresh water or plard effluent; (47) Sludge Gas Utilization. The process of using sewage gas for the purpose of heating buildings, driving engines, etc.; (48) Sludge Holding Tank (Aerated and ftlonaerated). A tank utilized for small wastewater treatment plants not containing a digester In which sludge may be kept fresh, and supernatant withdrawn prior to a drying method (Le. sludge drying beds); This may be done by adding a amall arnourd of air simply to keep the sludge fresh, but not necessarily an amourn that would be required to achieve stabilization of organic matter. A nonaoraled tank would simply be used to decant sludge prior to dewatering and would not allow long periods (several days of detention) wh hout resulting odor problems; (49) Sludge Incinerators. A furnace designed to bum sludge and to remove all moisture and combustible materials and reduce the sludge to a sterile ash; (50) Sludge Stabilization (Chemical or Thorma)). A process to make treated sludge less odorous and putrescible, and to reduce the pathogenic organism content; This may be done by pH adjustment, chlorine dosing, or by heat treatment; (51) Sludge Thickener. A biological treatment unit consisting of a material such as token stone or rock over which wastewater is distrbuted; A high rate trickling filter Is one which operated al between 10 and 30 mgd per acre. A low rate tricking fitter is one which is designed to operate at one to four mgid per acre; (60) Trickling Filter (Packed Tower). A plug flow type of operation in which wastewater flows down through successlve layers of medla or littrate material; Organic material Is removed corttlnually by the attire biological fixed growth In each successive layer. This method may produce 'secondary quality effluent, or may be adapted to produce a nitrified effluent; (61) Vacuum Filter, Centrifugets, or Filler Presses. Devices which are designed to remove excess water from either digested or undigested sludge prior to disposal or further treatment.
