Dear Dr. Bridges:
In accordance with the application which was received on May 25, 1989, we are
forwarding herewith Permit No. WQ0001766, dated May 26, 1989, to the Wake County Board of
Education for the operation of the subject wastewater pump and haul activities.
This permit shall be effective from the date of issuance until June 30, 1989, and shall be
subject to the conditions and limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are unacceptable to you,
you have the right to request an adjudicatory hearing upon written request within thirty (30) days
following receipt of this permit. This request must be, in the form of a written petition, conforming
to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative
Hearings, P.O. Drawer 11665, Raleigh, NC 27604. Unless such demands are made this permit
shall be final and binding.

For the discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North
Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
operation of a 5,400 GPD wastewater pump and haul activity which will consist of pumping
wastewater from the Vance Elementary School until repairs are completed on the low pressure pipe
disposal system. This wastewater will be pumped and hauled by B & D Septic Tank Service and
will be deposited for treatment in the Raleigh Neuse Wastewater Treatment Plant, pursuant to the
application received May 25, 1989, and in conformity with the project plan, specifications, and
other supporting data subsequently filed and approved by the Department of Natural Resources and
Community Development and considered a part of this permit.
This permit shall be effective from the date of issuance until June 30, 1989, and shall be
subject to the following specified conditions and limitations:
This perniit shall become voidable unless the facilities are operated in accordance with
the approved plans, specifications, and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described in
the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable.
5, The sewage and wastewater collected during this operation shall be adequately treated in the
Raleigh Neuse Wastewater Treatment Facility prior to final disposal.
6. This permit shall not be renewed or extended.
7. B & D Septic Tank Service is liable for any damages which occur as a result of a
spill.
8. No wastewater other than that from Vance Elementary School shall be included in this
pump and haul operation.
9. Adequate inspection, maintenance, and cleaning shall be provided by the Wake County
Board of Education to insure proper operation of these facilities.
10. In the event that these activities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take such immediate corrective action as may be
required by this Division, including the construction of additional or replacement
wastewater treatment or disposal facilities.
11- The Permittee shall maintain an accurate record of inspection and pumping activities
associated with this operation ( including frequency and amount ).
