Box 10 GRANITE PALLS, NC 28630 Subject: Permit No. WQ0001618 Town of Granite Falls Town of Granite palls Residuals Land Application Program Land Application of Residual Solids (503) Caldwell County Dear Mr. Hayes: In accordance with your modification request received April 18, 2007, we are forwarding herewith a modified Attachment A and B, certified August 17, 2007, to the Town of Granite Falls for the subject residuals land application program. This certification is being issued to modify Attachment A to: ! Approve the application of the Town of Granite Falls WTP alum sludge as part of the Town's residual land application program. This certification is being issued to modify Attachment B to: ♦ Include the dominant soil type for each approved application sites. This modified Attachment A and B shall be effective from the date of certification; shall void Attachment A and B of Permit No. WQ0001618 that was issued on April 21, 2006; and shall be subject to the conditions and limitations as specified therein and well as those specified in the above -referenced permit. As always, remember to take the time to review this modified Attachment A and B thoroughly, as some of the conditions contained therein may have been changed since the last certification. Note that this modified Attachment A and B are hereby incorporated by reference into Permit No. WQ0001618, issued on April 21, 2006. Please replace the existing Attachment A and B of Permit No. WQ0001618 that was issued on April 21, 2006, with this modified Attachment A and B. It is the Permittee's responsibility to ensure that inserting this modified Attachment A and B into Permit No. WQ0001618 to create a complete permit; making sure to retain all updated and existing vicinity and buffer maps for all land application sites. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, NC 27699-6714.
