In accordance with your modification request received on March 14, 2008, we are forwarding
herewith a modified Attachment A, certified April 7, 2008, to US MCAS — Cherry Point for the subject
treatment facilities and a program for the distribution of processed waste and residuals.
This certification is being issued to modify Attachment A to change monitoring for
Cadmium from quarterly to annually (see Attachment A).
Please replace the existing Attachment A of Permit No. WQ0001489 that was previously
certified on August 26, 2005 with this modified Attachment A. It is the Permittee's responsibility to
ensure that a complete permit is created by inserting this modified Attachment A into Permit No.
WQ0001489, issued on August 26, 2005.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right
to request an adjudicatory hearing upon written request within 30 days following receipt of this permit.
This request must be in the form of a written petition, conforming to Chapter 150B of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service Center,
Raleigh, North Carolina 27699-6714. Unless such demands are made, this permit shall be final and
binding.
