Dear Mr. Klein:
On October 20, 1989, the Division of Environmental Management issued the subject permit
to Burlington Industries, Inc. Upon review of this permit, the Division has determined that a
minor modification should be made to condition 111.3 of Permit No. WQ0001462, which was
issued on October 20, 1989; therefore, please note that the requirement for submitting records to
the Raleigh Regional Office in condition 111.3 is hereby deleted. This condition is now modified to
require that an annual summary report (three copies) of the records required in condition III.3 be
submitted on or before March 1 of the following year, by Burlington Industries, Inc., to the
following address:

Raleigh, NC 27626-0535
Additionally, any records (three copies) required by conditions III.1, I11.4, II1.5, and 11I.6 must
also be submitted to the above address by March I of the following year by Burlington Industries,
Inc. All other conditions and limitations expressed in Permit No. WQ0001462, which was issued
on October 20, 1989, remain unchanged.
If this modification is unacceptable to you, you have the right to request an adjudicatory
hearing upon written request within thirty days following receipt of this permit modification. This
request must be in the form of a written petition, conforming to Chapter 150B of the North
Carolina General Statutes, and filed with the Office of Administrative Hearings, P. O. Drawer
27447, Raleigh, N. C. 27611-7447. Unless such demands are made, this permit modification
shall be final and binding.
