Dear Mr, White:
In accordance with your application received on March 27,
1989, we are forwarding herewith Permit No. WQ 0001455, dated
May 8, 1989, to City of Charlotte Parks and Recreation
Department for the construction and operation of the subject
non -discharge type wastewater treatment facilities.
This Permit shall be effective from the date of issuance
until rescinded, and shall be subject to the conditions and
limitations as specified therein.
If any parts, requirements, or limitations contained in
this Permit are unacceptable to you, you have the right to an
adjudicatory hearing upon written request within 30 days
following receipt of this Permit. This request must be in the
form of a written petition, conforming to Chapter 150B of North
Carolina General Statutes, and filed with the Office of
Administrative Hearings, Post Office Drawer 11666, Raleigh,
North Carolina 27604.
If you wish to have the hearing before a Hearing Officer
with this Department, you must indicate in the petition that you
waive the right to have the contested case conducted by a
Hearing Officer in the Office of Administrative Hearings, and
wish to have the matter conducted in the Department of Natural
Resources and,Community Development. Unless such demands are
made this Permit shall be final and binding.

Please be advised that the Mooresville Regional Office, 919
North Main Street, Post Office Box 950, Mooresville, North
Carolina 28115 shall be notified at least 48 hours in advance of
backfilling of the installed subsurface disposal system so that
an "in --place" inspection can be conducted prior to the
facilities being covered.

For the Discharge of Sewage, Industrial Wastes, or other Wastes
In accordance with the provisions of Article 21 of Chapter
143, General Statutes of North Carolina as amended, and other
applicable Laws, Rules and Regulations
PERMISSION IS HEREBY GRANTED TO
City of Charlotte Parks & Recreation Department
FOR THE
construction and operation of a 1,200 gpd non -discharge
wastewater disposal system consisting of a 2,000 gallon septic
tank; a 2,000 gallon dosing tank with a 40 gpm siphon pump; a
10,800 square foot nitrification field with a 41" pressure
manifold connecting nine (9) 65' x 4" drain lines on one side and
nine ( 9 ) 85' x 4" drain lines on the other; and appurtenances to
serve an Environmental Center at Reedy Creek Park in Charlotte,
Mecklenburg County with no discharge of collected domestic
wastewater into the surface waters of the State, pursuant to the
application received on March 27, 1989, and in conformity with
the project plans, specifications, and other supporting data,
subsequently filed and approved by the Department of Natural
Resources and Community Development and considered a part of
this Permit.
This Permit shall be effective from the date of issuance
until rescinded, and shall be subject to the following specified
conditions and limitations:
1. This Permit shall become voidable unless the
facilities are constructed in accordance with the
approved plans, specifications and other supporting
data. Upon completion of construction of these
facilities, the Mooresville Regional Office, 919 North
Main Street, Post Office Box 950, Mooresville, North
Carolina 28115 (telephone number 704/663-1699) shall
be notified at least 48 hours in advance of
backfilling so that an "in-place" inspection can be
conducted prior to the facilities being covered.

2. This Permit is effective only with respect to the
nature and volume of wastes described in the
application and other supporting data.
3. The facilities shall be properly maintained and
operated at all times.
4. This Permit is not transferable.
5. This Permit shall become voidable in the event of
failure of the soil to adequately absorb the wastes,
and may be rescinded unless the facilities are
installed, maintained and operated in a manner which
will protect the assigned water quality standards of
the surface waters and ground waters.
6. In the event the facilities fail to perform
satisfactorily, the Permittee shall take such
immediate corrective action as may be required by the
Division of Environmental Management.
7. Sludge generated from this treatment system must be
disposed of in a manner as required by the Division of
Environmental Management to comply with North Carolina
General Statute 143215.1.
8. Diversion and/or bypassing of the treated and/or
untreated wastewater from this system is prohibited.
9. An acceptable reserve area, equal in size to the
approved disposal area shall be maintained at all
times.
10. The approved nitrification field site shall not be
disturbed, regraded or substituted without written
permission from the Mooresville Regional Supervisor.
11. The proposed subsurface disposal system shall be
located at least 100 feet from any public or private
water supply well.
12. Adequate inspection, maintenance, and cleaning shall
be provided by the Permittee to insure proper
operation of the subject facilities.
13. Any groundwater quality monitoring, as deemed
necessary by the Department, shall be provided by the
Permittee.
14. The issuance of the Permit shall not relieve the City
of Charlotte Parks and Recreation Department of the
responsibility for damages to surface and/or ground
waters resulting from the operation of this facility.
