Dear Mr. Crump:
In accordance with your application received March 17, 1989, we are
forwarding herewith Permit No. WQ0001386, dated March 28, 1989, to WACC
Associates, Inc. for the construction and operation of the subject septic tank
subsurface -disposal system.
This permit shall be effective from the date of issuance until March 31, 1994,
and shall be subject to the conditions and limitations as specified therein.
Issuance of this permit shall void Permit No. 6139 issued September 4, 1980.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request must
be in the form of a written petition, conforming to Chapter 150E of Forth Carolina
General Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this
permit shall be final and binding.

For the discharge of Sewage, Industrial Wastes, or. Other Wastes
In accordance with the provisions of Article 21. of Chapter 143, General. Statutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO
WACC Associates, Inc.

continued operation of a 600 GPD non -discharge type wastewater treatment facility
consisting of a 1,500 gallon capacity septic tank and approximately 1,050 linear feet
of 4 -inch drain pipe and 3,150 square feet of drain field to serve Sparkling Clean
Car Wash with no discharge of wastewater to the surface waters of the State, pursuant
to the application received March 17, 1989, and in conformity with th project plan,
specifications, and other supporting data. subsequently filed and approved by the
Department of Natural. Resources and Community 'Development and considered a part of
this permit.
This permit shall be effective from the date of issuance until March 31, 1994,
and shall be subject to the following specified conditions and limitations:
1.. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable.
5. Upon the availability of a municipal or regional sewerage collection system, the
subject wastewater treatment system shall be abandoned and all wastewater
discharged into the munici.pal or regional sewerage system.
6. This permit shall become voidable in the event of failure of the soil to
adequately absorb the wastes and may be rescinded unless the facilities are
1 installed, maintained, and operated i.n a manner which will protect the assigned
water quality standards of the s"r.face waters and ground waters.
In the event that the facilities fail to perform satisfactorily, including the
I creation of nuisance conditions, the Permittee shall take such immediate
corrective action as may be required by this Division, including the construction
of additional or replacement wastewater treatment or disposal facilities.
8. Any monitoring deemed necessary by the Division of Environmental Management to
insure surface and ground water protection will be established and an acceptable
sampling reporting schedule shall be followed.
9. The sludge generated from these treatment facilities must be disposed of in
accordance with General Statute 1.43-213.1 and in a manner approvab.e by the North
Carolina Division of Environmental Management.
10, The treatment facility shall be effectively maintained and operated at all times
so that there is no discharge to the surface waters, nor any contamination. of
ground waters which will render them unsatisfactory for normal use.
11. Diversion or bypassing of the untreated wastewater from the treatment facility,
is prohibited.
1.2. The Permittee, at least six (6) months prior to the expiration of this permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the facilities described therein, and if warranted, will
extend the Permit .for such period of time and under such conditions and
limitations as it may deem appropriate.
'I. The issuance of this permit shall not relieve the Permittee of the responsibility
for damages to surface or groundwaters resulting from the operation of this
facility,
14. The facilities shill be effectively maintained and operated as a non -discharge
system to prevent the discharge of any wastewat.er resulting from the operation of
this facility.
1.5_ Solids, sludges, or other pollutants removed or resulting from the wastewater
storage facilities shall be contained and disposed of in such a manner as to
prevent any contamination of the surface or groundwaters of the State.
1.6. Issuance of this permit shall void Permit No. 6139 issued September 4, 1980.
