Box 1029 Lillington, NC 27546 SUBJECT: Permit No. WQ0001345 Harnett County Schools Western Harnett Middle School Wastewater Treatment and Disposal Facility Harnett County Dear Mr. Wortman: In accordance with your application received March 10, 1989, we are forwarding herewith Permit No. WQ0001345, dated April 17, 1989, to Harnett County Schools for the construction and operation of the subject wastewater treatment and disposal facility. This permit shall be effective from the date of issuance until May 1, 1994, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150E of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this permit shall be final and banding. Box 27687, Raleigh, North Carolina 27611-7687 Telephone 919.733-7015 An Equal Opportunity Affirmative Action Employer NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT RALEIGH P E R M I T For the discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO Harnett County Schools Harnett County FOR THE construction and operation of a .16,500 GPD wastewater treatment and disposal facility consisting of 1.3,500 gallon baffled septic tank with dosing chamber, three (3) 94• GPM triplex submersible pumps with highwater alarms, and approximately 14,000 linear feet of nitrification trench (three fields with thirteen. 120 foot lines each), and equal replacement area to serve the Western Harnett Middle School with no discharge of wastewater to the surface waters pursuant to the application received March 10, 1989, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until May 1, 1994, and shall. be subject to the following specified conditions and limitations: I. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the approved plans and specifications. Mail the certification to the Permits and Engineering Unit, P.O. Box 27687, Raleigh, NC 27611-7687, 2. This permit shall become voidable unless the facilities are constructed in accordance with the approved pians, specifications, and other supporting data. 3. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 4. The facilities shall be properly maintained and operated at all times. 5. This permit is not transferable. } 6. Upon the availability of a municipal or regional sewerage collection system, the subject wastewater treatment facility shall be abandoned and all wastewater discharged into the municipal or regional sewerage system. 7. This permit shall become voidable in 'the event of failure of the soil to adequately absorb the wastes and may be rescinded unless the facilities are installed, maintained, and operated in a manner which will protect the assigned water quality standards of the surface waters and ground waters. 8. in the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or disposal facilities. 9. The Fayetteville Regional Office, phone no. (919) 486-1451 shall be notified at least twenty-four (24) hours in advance of backfilling of the installed system so that an in-place inspection can be made. Such notificatioii to the regional supervisor shall. be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State holidays. 10. The sludge generated from these treatment facilities must be disposed of in accordance with General Statute 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. 11. Diversion or bypassing of the untreated wastewater from the treatment facility is prohibited. 12. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the .Permit for such period of time and under such conditions and limitations as it may deem appropriate. 13. The application rates shall not exceed 1.2 GPD per square foot. 14. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 15. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 16. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal maintenance is being performed. 17. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 18. An acceptable reserve area, equal in size to the approved disposal area, shall be maintained at all, times. p 19. A set of approved pians and specifications for the subject project must be retained by the applicant for the life of the project. 20. Any groundwater quality monitoring, as deemed necessary by the Division, shall be provided. 21. The Western Harnett Middle School shall install water conservation devices to reduce wastewater volume tributary to the subject treatment and disposal facility.
