In accordance with your permit modification application package received on January 18, 2007 as
well as the additional information received on February 20, 2007, we are forwarding herewith a modified
Attachment A and Attachment B, certified August 14, 2007 to Town of Spring Lake for the subject
residuals land application program.
This certification is being issued to modify Attachment A to increase the maximum dry tons per
year to 287 dry tons.
The Attachment B certification is being issued to approve new land application sites and modify
the existing site. Attachment B has been modified to add land application sites owned by Tom Brooks in
Cumberland County. These sites total 123.8 net acres. The present site owned by the Town of Spring
Lake is decrease to 8 net acres. With these changes to Attachment B, the Town of Spring Lake Residuals
Land application Program now has 131.8 acres of approved land application sites.
Please replace the existing Attachment A and Attachment B of Permit No. WQ0001086 that
were previously certified on February 8, 2006, with these modified Attachment A and Attachment
B. Note that only the vicinity and buffer maps for any new and modified land application sites are
attached to this modified Attachment B. It is the Permittee's responsibility to ensure that a complete
permit is created by inserting these modified Attachment A, Attachment B and vicinity and buffer maps
into Permit No. WQ0001086, issued on February 8, 2006, and making sure to retain any vicinity and
buffer maps for any existing and unmodified land application sites.

These modified Attachment A and Attachment B shall be effective from the date of certification;
shall void Attachment A and Attachment B of Permit No. WQ0001086, that was previously certified on
February 8, 2006; and shall be subject to the conditions and limitations as specified therein and well as
those specified in the above -referenced permit. As always, remember to take the time to review these
modified Attachment A and Attachment B thoroughly, as some of the conditions contained therein
may have been changed since the last certification. Note that these modified Attachment A and
Attachment B are hereby incorporated by reference into Permit No. WQ0001086, issued on February 8,
2006.
If any parts, requirements, and/or limitations contained in these certifications or modified
Attachment A and Attachment B are unacceptable, you have the right to request an adjudicatory hearing
upon written request within 30 days following receipt of the certification. This request shall be in the
form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed
with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-
6714. Unless such demands are made, this permit shall be final and binding.
