Pump and Haul Alamance County Dear Mr. McKeller: In accordance with your application received January 6, 1989, we are forwarding herewith Permit No. WQ0001027, dated February 1, 1989, to Glen haven Mills, Inc. for the construction and operation of the subject wastewater pump and haul disposal system. This permit shall be effective from the date of issuance until August 2, 1989, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit.. This request must be in the form of a written petition, conforming to Chapter 1508 of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. sinless such demands are made this permit shall be final and binding. One (1) set of approved plans and specifications is being forwarded to you. Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO Glen Raven Mills, Inc. Alamance County FOR THE construction. and operation of a 1.5,000 GPD pump and haul disposal system from Glen Raven Mills, Incorporated's Altamahaw Plant to the Glen Raven. Plant and the discharge of the collected industrial wastewater into the the City of Burlington's existing sewerage system pursuant to -the application received January b, 1989, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until August 2, 1989, and shall be subject to the following specified conditions and limitations: 1. This permit shall become voidable unless the facilities are operated in accordance with the approved plans, specifications and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities shall be properly maintained and operated at all times. 4. The sewage and wastewater collected by this system shall be treated in the East Burlington Wastewater Treatment Plant prior to being discharged into the receiving stream. 5. This permit is not transferable. b. This permit shall become voidable unless the agreement between Glen Raven Incorporated and the City of Burlington for the collection and final treatment of wastewater is in full force and effect. 7. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate 1 corrective action as may be required by this Division. 8. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 9. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 10. Failure of the Permittee to properly operate this system is subject to a penalty up to $10,000 per day. 11. No wastewater other than that from the Glen Raven Mills' Altamahaw Plant shall be included in this pump and haul permit. 12. The Permittee is liable for any damages caused by a spill or failure of the pump and haul operation. 13. The pump and haul shall be discontinued upon he availability of sewer line service or August 1, 1989, whichever comes first. 14. The average daily flow of wastewater pumped and hauled shall not exceed 15,000 GPD. 15. This permit shall not be renewed or extended. Permit issued this the lst day of February, 1989.
