In accordance with an oral request received February 2, 2015, from Will Hart of the Washington
Regional Office, we are forwarding herewith corrected Permit No. WQ0001026 issued January 23, 2015.
The corrections to the subject permit are as follows:
1. The Regional Office to contact is the Washington (and not the Fayetteville) RO on page 1 of
the letter.
2. The effective date of the permit shall be January 23, 2015, and not May 1, 2015.
3. The end date of the permit shall be December 31, 2019, and not April 30, 2020.
Please replace Pages 1 and 2 of the Cover Letter and Pages 1 and 2 of Permit No.
WQ0001026 issued January 23 with the enclosed corrected pages.

Dear Mr. Waters:
In accordance with your permit renewal and major modification request received December 3,
2014, we are forwarding herewith Permit No. WQ0001026 dated January 23, 2015, to City of
Washington for the continued operation of the subject residuals management program.
Modifications to the subject permit are as follows:
1. Removal of Fields from Attachment B of the May 9, 2006 permit and February 8, 2007
modified permit: NC-1317-202-02, NC-BF-207-03, NC-BF-207-04, NC-PI-03-02, NC-PI-I1-
01 through 09, NC-PI-164-01 through 05, and NC-PI-200-05 through 11.
2. Addition of Fields to Attachment B: NC-BF-210-01 through 16.
3. Various changes to field descriptions including landowner, dominant soil series, and acreages
to Fields in Attachment B.
This permit shall be effective from January 23, 2015, until December 31, 2019, shall void Permit
No. WQ0001026 issued May 9, 2006 and modified February 8, 2007, and shall be subject to the
conditions and limitations as specified therein. Please pay particular attention to the monitoring
requirements listed in Attachments A and B for they may differ from the previous permit issuance.
Failure to establish an adequate system for collecting and maintaining the required operational
information shall result in future compliance problems.
Please note the following permit conditions are new since the last permit issuance:
1. Condition 1.2: By August 1, 2015, the Permittee shall notify the Washington Regional Office
48 hours prior to each land application of residuals.
2. Condition I.3: By November 1, 2015, the Permittee shall be in compliance with signage
requirements.
3. Condition 11.5: This is similar to Conditions I.6 and 111.8 of the 2006 permit, but methods for
determining compliance with Cumulative Pollutant Loading Rates (CPLR) are specified.
4. Condition H.9: Similar to Condition 13 of 2006 permit, but methods of Realistic Yield
Expectations (RYE) from Plant Available Nitrogen (PAN) specified.

5. Condition 111.2: Same as Condition I.10 and I.11 of the 2006 permit, except now 48-hour
notice rather than 24-hour notice provided to local government official and Washington
Regional Office for initial land application of residuals.
6. Condition 111.3: The Washington Regional Office shall be informed 24 hours prior to each
land application of residuals.
7. Condition III.4: Requirement for an O&M Plan.
8. Condition IH.6: Similar to Condition II.4 of 2006 permit, but now copy of O&M Plan must
also be on site during application.
9. Condition IH.I La, b, j and k: Bulk residuals shall not be land applied if an endangered
species adversely affected, prolonged nuisance conditions are created, the distance is too
small between the surface and the seasonal high water table and/or bedrock, respectively.
10. Condition III.13: Signage requirements to prohibit public access.
11. Condition V.1: Permittee must provide adequate inspection and maintenance.
12. Condition VI.6: SimiIar to Condition VIA of 2006 permit, but now 60-day notice given of
revocation.
13. Condition VI.7: Expansion of residuals program not allowed if Permittee has exhibited
improper conduct unless a variance granted.
Please note that on September 1, 2006, North Carolina Administrative Code Title 15A
Subchapter 02T — Waste not Discharged to Surface Waters was adopted. The previous permit was issued
before this date. Accordingly, this permit incorporates the requirements of these rules which are reflected
in the changes listed above.
If any parts, requirements or limitations contained in this permit are unacceptable, the Permittee
has the right to request an adjudicatory hearing upon written request within 30 days following receipt of
this permit. This request shall be in the form of a written petition, conforming to Chapter 150B of the
North Carolina General Statutes, and filed with .the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Unless such demands are made, this permit shall be final and
binding.

LAND APPLICATION OF CLASS B RESIDUALS PERMIT (NON -DEDICATED)
In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO
City of Washington
Beaufort County
FOR THE
continued operation of a residuals management program for City of Washington and consisting of the
land application of Class B residuals generated by the approved facilities listed in Attachment A to the
approved sites listed in Attachment B with no discharge of wastes to surface waters, pursuant to the
application received December 3, 2015, and in conformity with other supporting data subsequently filed
and approved by the Department of Environment and Natural Resources and considered a part of this
permit. The use and disposal of residuals are regulated under Title 40 Code of Federal Regulations Part
503. This permit does not exempt the Permittee from complying with the federal regulations.
This permit shall be effective from January 23, 2015, until December 31, 2019, shall void Permit No.
WQ0001026 issued May 9, 2006, and shall be subject to the following specified conditions and
limitations:
I. SCHEDULES
1. No later than six months prior to the expiration of this permit, the Permittee shall request renewal of
this permit on official Division forms. Upon receipt of the request, the Division will review the
adequacy of the facilities described therein, and if warranted, will renew the permit for such period of
time and under such conditions and limitations as it may dccm appropriate. Please note Rule 15A
NCAC 02T .0105(d) requires an updated site map to be submitted with the permit renewal
application. [I5A NCAC 02T .0108(bx2), 02T .105(d), 02T .0109]
2. The Permittee shall be in full compliance with the regional office notification requirements
established in Condition 111.3. by August 1, 2015. [15A NCAC 02T .0108(b)(2)]
3. The Permittee shall be in full compliance with the signage requirements established in Condition
III.13. by November 1, 2015. [15A NCAC 02T .0108(b)(2)]

H. PERFORMANCE STANDARDS
The subject residuals management program shall be effectively maintained and operated at all times
so there is no discharge to surface waters, nor any contravention of groundwater or surface water
standards. In the event the facilities fail to perform satisfactorily, including the creation of nuisance
conditions due to improper operation and maintenance, the Permittee shall immediately cease land
applying residuals to the site, contact the Washington regional office supervisor, and take any
immediate corrective actions. [G.S. 143-215.11
2. This permit shall not relieve the Permittee of responsibility for damages to groundwater or surface
water resulting from the operation of this residuals management program. [15A NCAC 02B .0200,
02L .0100]
3. OnIy residuals generated by the facilities listed in Attachment A are approved for land application in
accordance with this permit. [G.S. 143-215.1]
4. Only the sites listed in Attachment B are approved for residuals land application. [G.S. 143-215.1]
5. Pollutant concentrations in residuals applied to land application sites listed in Attachment B shall not
exceed the following Ceiling Concentrations (i.e., dry weight basis) or Cumulative Pollutant

The Permittee shall determine compliance with the CPLRs using one of the following methods:
a. By calculating the existing cumulative level of pollutants using actual analytical data from all
historical land application events of residuals, or
b. For land where residuals application have not occurred or for which the required data is
incomplete, by determining background concentrations through representative soil sampling.
6. Residuals that are land applied shall meet Class B pathogen reduction requirements in 15A NCAC
02T .1106 (a) and (b)(2). Exceptions to this requirement shall be specified in Attachment A. [15A
NCAC 02T .1106]
7. Biological residuals (i.e. residuals generated during the treatment of domestic or animal processing
wastewater, or the biological treatment of industrial wastewater, and as identified in Attachment A)
that are land applied shall meet one of the vector attraction reduction alternatives in 15A NCAC 02T
.1107(a). Exceptions to this requirement shall be specified in Attachment A. [15A NCAC 02T .1107]
8. When land applying residuals to any land application site, the following buffer zones shall be
maintained at all times:
