Recycle Wastewater Treatment Facility Surry County Dear Mr. Rose: In accordance with your application received January 5, 1989, we are forwarding herewith Permit No. WQ0001025, dated March 28, 1989, to Carl. Rose and Sons, Inc. for the continued operation of the subject wastewater treatment facility. This permit shall be effective from the date of issuance until April 1, 1994, shall void Permit No. 5227R and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150E of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this permit shall be final and binding. One (1) set of approved plans and specifications is being forwarded to you. Surry County FOR THE continued operation of three settling ponds with a 400 GPM recirculating pump to serve Carl Rose and Sons, In.c..'s asphalt batch plant with no discharge of wastewater to the surface waters, pursuant to the application received January 5, 1989, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until April 1, 1994, and shall be subject to the following specified conditions and limitations: 1. This permit shall void Permit No. 5227R. 2. This permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. • 3. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. • 4. The facilities shall. be properly maintained and operated at all times. s 5. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective acti_oii as may be required by this Division, including the construction of additional or replacement wastewater treatment or disposal facilities. 0 6. This permit is not trans Eerable. 7. Tho sludge gene -rated from these treatment f.aci.11.L-ies must: be disposed of in accordance with Gener.a.l. Statute 143-215.1 and in a manner approvable, by the North Carolina Division. of Environmental Management. 8. Satisfactory controls shall be provided to contain the accidental discharge or spillage of hazardous or toxic substances either stored or being used on the project site. 9. Diversion or bypassing of the untreated wastewater from the treatment facility is prohibited. 10. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate. ® l.l. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or grou.ndwaters resulting from the operation of this facility. Qk13. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 40 14. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 15. Three months prior to anticipated abandonment of the recycle wastewater treatment facilities. The Winston-Salem Regional Office shall be notified and approval of the disposal of residual wastewater and sludge must be obtained. 16. Within 60 days of permit issuance a sample shall. be obtained of the typical wastewater which is to be discharged into the settling ponds. The iastewater sample should be obtained from a point in the system prior to discharge into the first settling basin, and analyzed for the following parameters: I. Purgeable Halocarbon.s (EPA Method 601) 2. Pur.geable Aromatics (EPA Method 602) 3. Aldehydes (formaldehyde, isobutyr.aldehyde, n-butyraldehyde and isovaleraldehyde) The analytical methods used for. (3) above should be the appropriate EPA method or other equivalent method having the lowest detection limit, The results of the sampling and analysis shall bp sent to the N.C. Division of EnvironmenL-al Management, Groundwater Section, within 30 days of sample collection. 17. Any groundwater quality monitoring, as deemed necessary by the Division, shall be provided.
