In accordance with your application received January 15, 1999, we are forwarding herewith Permit No.
WQ0000962, dated May 7, 1999, to Culp Ticking (A Division of Culp, Inc.) for the continued operation of the
subject wastewater recycle system and abandonment/closure of two existing lagoons.
This permit shall be effective from the date of issuance until April 30, 2004, shall void Permit No.
WQ0000962 issued May 8, 1998, and shall be subject to the conditions and limitations as specified therein. Please
pay particular attention to the groundwater monitoring requirements and lagoon abandonment/closure requirements
provided for by Conditions Nos. 21 and 22, respectively, of this permit. Note that some of these requirements
differ from those in previously -issued permits. Failure to establish an adequate system for collecting and
maintaining the required operational information will result in future compliance problems.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to
request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This
request must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes,
and filed with the Office of Administrative Hearings, P.O. Drawer 27447, Raleigh, NC 27611-7447. Unless such
demands are made this permit shall be final and binding.

other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
continued operation of a 130,000-GPD wastewater recycle system consisting of a bar screen, a 3,650-gallon pump
tank with high water alarms and large solids removal (two Gorman -Rupp trash pumps with a float level switch), a
27,700-gallon equalization tank with a three -HP mixer, wastewater equalization and pH adjustment (a 100,000-
gallon equalization tank, floating downdraft mixer, acid metering pump, pH analyzer/controller, pH recirculation
pump, and appurtenances), dissolved air flotation system (a 400-GPM stainless steel DAF unit, a mixing pipe
flocculator with chemical injection, recirculation pump, stainless steel sparge tank and distribution manifold,
stainless steel sludge scraper, transfer pump, automatic DAF blowdown valve, magnetic flow meter, and
appurtenances), polymer containment mixing and metering (caustic pump, pH analyzer/controller, coagulant
metering gear pump, flocculant mix tank, flocculant tank mixer, flocculant metering gear pump), a 200-GPM pre -
assembled ClaRite Diatomaceous Earth Pressure Filter Unit with a 5,000-gallon filter feed pump station with 140-
GPM duplex pumps and a 45-SCFM pneumatic system, a 8,000-gallon sludge storage tank, a 0.5-meter belt filter
press, a 100,000-gallon finished water storage tank, an automatic control system, and all the necessary pipes,
valves, and appurtenances required for the proper operation of the system.
Approval is also given for the proper abandonment and closure of an existing 0.271-million gallon Iagoon and a
1.35-million gallon lagoon. Please note Condition No. 22 for the procedures for proper lagoon closure.
The above listed system serves Culp Ticking (A Division of Culp, Inc.), with no discharge of wastes to the surface
waters, pursuant to the application received January 15, 1999, and in conformity with the project plan,
specifications, and other supporting data subsequently filed and approved by the Department of Environment and
Natural Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until April 30, 2004, shall void Permit No.
WQ0000962 issued May 8, 1998, and shall be subject to the following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of
this permit, the approved plans and specifications, and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described in the application and
other supporting data.
The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is
a name change of the Permittee, a formal permit request must be submitted to the Division of Water Quality
(Division) accompanied by documentation from the parties involved and other supporting materials as may
be appropriate. The approval of the request will be considered on its merits and may or may not be approved.
5. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the
Permittee shall take immediate corrective action, including those actions that may be required by this
Division, such as the construction of additional or replacement treatment or disposal facilities.
6. The residuals generated from these treatment facilities must be disposed in accordance with General Statute
143-215.1 and in a manner approved by the Division.
7. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or
groundwaters resulting from the operation of this facility.
The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge
of any wastewater resulting from the operation of this facility.
Diversion or bypassing of untreated wastewater from the treatment facilities is prohibited.
10. Any monitoring deemed necessary by the Division to insure surface and ground water protection will be
established and an acceptable sampling reporting schedule shall be followed.
11. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation
of the subject facilities.
12. The Permittee or his designee shall inspect the wastewater recycle facilities to prevent malfunctions and
deterioration, operator errors and discharges which may cause or lead to the release of wastes to the
environment, a threat to human health, or a nuisance. The Permittee shall maintain an inspection log or
summary including at least the date and time of inspection, observations made, and any maintenance, repairs,
or corrective actions taken by the Permittee. This log of inspections shall be maintained by the Permittee for
a period of three years from the date of the inspection and shall be made available to the Division or other
permitting authority, upon request.
13. Any duly authorized officer, employe. or representative of the Division mav, upon presentation of
credentials, enter and inspect any property, premises or place on or related to the recycle system at any
reasonable time for the purpose of determining compliance with this permit, may inspect or copy any records
that must be maintained under the terms and conditions of this permit, and may obtain samples of
groundwater, surface water, or leachate.
14. The annual administering and compliance fee must be paid by the Permittee within 30 days after being billed
by the Division. Failure to pay the fee accordingly may cause the Division to initiate action to revoke this
permit as specified by 15A NCAC 2H .0205 (c)(4).
15. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to an
enforcement action by the Division in accordance with North Carolina General Statute 143-215.6A to 143-
215.6C.
16. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules,
regulations, or ordinances which may be imposed by other government agencies (local, state, and federal)
which have jurisdiction.
17. A set of approved plans and specifications for the subject project must be retained by the Permittee for the
life of this project.
18. Noncompliance Notification:
The Permittee shall report by telephone to the Winston-Salem Regional Office, telephone number (336) 771-
4600, as soon as possible, but in no case more than 24 hours or on the next working day following the
occurrence or first knowledge of the occurrence of any of the following:
a. Any occurrence at the wastewater treatment facility which results in the treatment of significant amounts
of wastes which are abnormal in quantity or characteristic, such as the dumping of the contents of a basin
or tank, the known passage of a slug of hazardous substance through the facility, or any other unusual
circumstances;
b. Any process unit failure, due to known or unknown reasons, that renders the facility incapable of
adequate wastewater treatment, such as mechanical or electrical failures of pumps, aerators, compressors,
etc.;
Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass directly to
receiving waters without treatment of all or any portion of the influent to such station or facility; or
d. Any time that self -monitoring information indicates that the facility is not in compliance with its permit
limitations.
Persons reporting such occurrences by telephone shall also file a written report in letter form within five days
following first knowledge of the occurrence. This report must outline the actions taken or proposed to be
taken to ensure that the problem does not recur.
19. Upon classification of the facility by the Water Pollution Control Systems Operators Certification
Commission (WPCSOCC), the Permittee shall employ a certified wastewater treatment plant operator to be
in responsible charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of
the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment
facilities by the WPCSOCC. The Permittee must also employ a certified back-up operator of the appropriate
type and grade to comply with the conditions of I5A NCAC 8A .0202. The ORC of the facility must visit
each Class I facility at least weekly and each Class 11, 111, and IV facility at least daily, excluding weekends
and holidays, and must properly manage and document daily operation and maintenance of the facility and
must comply with all other conditions of 15A NCAC 8A .0202.
20. The Permittee, at least six months prior to the expiration of this permit, shall request its extension. Upon
receipt of the request, the Commission will review the adequacy of the facilities described therein, and if
warranted, will extend the permit for such period of time and under such conditions and limitations as it may
deem appropriate.
21. Groundwater Monitoring Requirements:
a. Existing groundwater monitoring wells W-101, W-102, W-103, MW-107, MW-108, and MW-109 shall
be renamed, for the purposes of sampling and reporting, as MW-1, MW-2, MW-3, MW-4, MW-5, and
MW-6, respectively,
b. Existing groundwater monitoring wells MW-I, MW-2, and MW-3 shall be sampled every March, July,
and November for the parameters listed below:
Water Level pH
Iron Total Dissolved Solids (TDS)
Manganese Total Organic Carbon (TOC)
Nitrate Chloride
Volatile Organic Compounds (VOCs) - in November only
Existing groundwater monitoring wells MW-4, MW-5, and MW-6 shall be sampled annually for the
parameters listed below:
Chlorides pH
Iron TDS
Manganese Nitrate
VOCs shall be tested by one of the following methods:
i. Standard Method 6230D, PQL at 0.5 µg/L or Iess,
ii. Standard Method 6210, PQL at 0.5 µg/L or less,
iii. EPA Method 8021, Low Concentration, PQL at 0.5 µg/L or less,
iv. EPA Method 8260, Low Concentration, PQL at 0.5 µg/L or less, or
V. Another method with prior approval by the Groundwater Section Chief
Any method used for testing VOCs must meet the following qualifications:
i. The laboratory must be Division -certified to run the method used.
ii. The method used must, at a minimum, include all the constituents listed in Table Vlff of Standard
Method 6230D.
iii. The method used must provide a PQL of 0.5 gg/L or less, which must be supported by laboratory
proficiency studies as required by the Division's Laboratory Certification Unit. Any constituents
detected above the MDL, but below the PQL of 0.5 µg/L must be qualified (estimated) and
reported.
If any VOCs are detected by the methods listed, then the Winston-Salem Regional Office Groundwater
Supervisor must be contacted immediately at telephone number (336) 771-4600 for further instructions
regarding any additional follow-up analyses required. The results of all initial and follow-up analyses
must be submitted simultaneously.
The measurement of water levels must be made prior to sampling for the remaining parameters. The
depth to water in each well shall be measured from the surveyed point on the top of casing.
If a TOC concentration greater than 10 mg/L are detected in any downgradient groundwater monitoring
well, additional sampling and analysis must be conducted to identify the individual constituents
comprising the elevated TOC concentration. If the TOC concentration, as measured in the background
groundwater monitoring well exceeds 10 mg/L, this concentration will be taken to represent the
naturally -occurring TOC concentration. Any exceedanees of this naturally -occurring TOC concentration
in the downgradient groundwater monitoring wells shall be subject to the additional sampling and
analysis as described above.
The results of the sampling and analysis must be received on Form GW-59 (Groundwater Quality
Monitoring: Compliance Report Form) by the Groundwater Section, Permits and Compliance Unit, P.O.
Box 29578, Raleigh, North Carolina, 27626-0578 on or before the last working day of the month
following the sampling month.
c. Any additional groundwater quality monitoring, as deemed necessary by the Division, shall be provided_
d. The COMPLIANCE BOUNDARY for the recycle system is specified by regulations in 15A NCAC 2L,
Groundwater Classifications and Standards. The Compliance Boundary for systems individually
permitted after December 31, 1983 is established at either (1) 250 feet from the waste treatment area, or
(2) 50 feet within the property boundary, whichever is closest to the waste treatment area_ An
exceedance of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to
immediate remediation action in addition to the penalty provisions applicable under General Statute 143-
215.6A(a)(1).
In accordance with 15A NCAC 2L, a REVIEW BOUNDARY is established around the recycle system
midway between the Compliance Boundary and the perimeter of the waste treatment area. Any
exceedance of standards at the Review Boundary shall require remediation action on the part of the
Permittee.
22. Lagoon Closure Procedures:
a. The existing unlined earthen lagoons which are no longer used as part of the recycle system shall be
abandoned in accordance with methods and procedures described below and as approved by the Winston-
Salem Regional Office within 90 days of permit issuance. Documentation in the form of a Certification,
signed and sealed by a North Carolina -licensed professional engineer, shall be provided to the Division
attesting to the proper and permanent closure of these lagoons within 60 days of closure completion. Mail
the Certification to the Non -Discharge Permitting Unit, P.O. Box 29535, Raleigh, NC 27626-0535.
b. The following guidelines shall be met for closure of the two existing lagoons:
Sample the sludge in each lagoon in three locations (i.e., three sludge samples per lagoon).
ii. The following analytical procedures should be run on the sludge samples:
A. Toxicity Characteristic Leaching Procedure (TCLP) SW-846 EPA Method 1331 for
voIatiles, semi-volatiles, and metals,
B. Ignitability SW-846 EPA Method 1010/1020,
C. Reactivity EPA OSW, and
D. Corrosivity EPA Method 9045.
iii. If the lagoon does not have a liner, then the underlying soil material should be sampled in three
locations and analyzed for TLCP procedures as listed above. If the lagoon has a natural compacted
clay liner, then the clay liner should be sampled in three locations and analyzed for the TCLP
procedures as listed above. If the Iagoon has a synthetic liner, the soil material underneath the
synthetic liner should be sampled in three locations and analyzed for the TCLP procedures as listed
above. From a field sampling point of view, the sludge and underlying clay or underlying soil
material can be collected using a core barrel sampler for the shallow soils, where the core barrel is
driven into the sludge/clay/soil matrix and later extracted.
iv. If the sludge/clay/underlying soil material has TCLP concentrations that are above the TCLP
regulatory limits, the material is considered hazardous, the Hazardous Waste Section of the
Division of Waste Management should be contacted, and the material needs to be excavated.
If the sludge/clay/underlying soil material has TCLP concentrations that are below the TCLP
regulatory limits and below Groundwater 2L standards then the material may be left in place and
the lagoon closed without excavating the sludge.
vi. If the sludge/clay/underlying soil material has TCLP concentrations that are below the TCLP
regulatory limits but are above the Groundwater 2L standards, then there are two options as
follows:
A. Excavate the material that is above the Groundwater 2L standards, then proceed with
lagoon closure.
B. Perform predictive calculations that demonstrate that the vertical leaching of the
contaminant in question in the material will not cause an exceedance of Groundwater 2L
standards in the underlying surficial groundwater saturated zone. This calculation would
probably require the need of site characterization of the subsurface hydrogeologic
parameters of concern. The predictive calculation would need to consider vertical leaching
and subsequent mixing in the groundwater saturated zone.
vii. Once it is decided whether the sludge/underlying soil material can remain in place or needs to be
excavated, the following steps shall be followed:
A. Remove all water from the lagoons and dispose of properly,
B. Remove all inflow piping, etc. to the lagoons,
C. If the liner (clay or synthetic) is being left in place, it must be breached or ripped up to
allow water to infiltrate past the liner,
D. Remove the earthen berms and grade the area to match the surrounding topography, in
order to prevent water from accumulating and ponding, and
E. Add a vegetative cover to prevent soil erosion.
viii. If any land disturbing activity associated with the above procedures will affect more than one acre
of land, an erosion and sedimentation control plan must be approved by the Land Quality Section
of the Division of Land Resources. The North Carolina General Statute 113A-57(4) requires that
this plan be submitted at lease 30 days prior to beginning the land disturbing activity.

The lollowing delinitions shalt apply throughout this Subchapter.

method wilt have a flow rate varying from two to eight gallons per minute per square foot end may be either upflow or downflow carbon beds. Carbon may or

(2) Awaled Lagoons. A basin In which all solids are malntalned in suspension and by which biological oxidation or organic matter Is reduced through artificially
acaeleraled transfer of oxygen on a flow -through basis;
(3) Aeration A process of bringing about Intlmate contact between tit or high purity oxygen In a Liquid by spraying, aghation or d7nuslon;(3a) Extended
Aeration. An activated sludge process utilizing a minimum hydraulic deteniion lima of 13 hours.
