Dear Mr. Hardison:
In accordance with your request received December 5, 1988, we are forwarding
herewith Permit No. WQ0000902, dated May 17, 1989, to Washington Packing Company,
Inc. for the continued operation of the subject wastewater treatment facilities.
This permit shall be effective from the date of issuance until April 30, 1994,
shall void Permit No. 3195R2, which was issued on June 7, 1987, and shall be subject
to the conditions and limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request must
be in the form of a written petition, conforming to Chapter 1508 of North Carolina
General. Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer 11666, Raleigh, North Carolina 2760+. Unless such demands are made this
permit shall be final and binding.

For the discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
continued operation of an 8,000 GPD closed system industrial wastewater treatment
facility consisting of a grease trap and a 0.5 acre holding lagoon with no discharge
to the surface waters of the State, pursuant to the request received December 5,
1988, and in conformity with the project plan, specifications, and other supporting
data subsequently filed and approved by the Department of Natural Resources and
Community Development and considered a part of this permit.
This permit shall be effective from the date of issuance until April 30, 1994,
shall void Permit No. 3195R2, which was issued on June 7, 1987, and shall be subject
to the following specified conditions and limitations:
1. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
2. The facilities shall be properly maintained and operated at all times.
3. This permit is not transferable.
4. In the event that the facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take such immediate
corrective action as may be required by this Division, including the construction
of additional or replacement wastewater treatment or disposal facilities.
5. Any monitoring deemed necessary by the Division of Environmental Management to
insure surface and ground water protection will be established and an acceptable
sampling reporting schedule shall be followed.
6. The sludge generated from these treatment facilities must be disposed of in
accordance with General Statute 143-215.1 and in a manner approvable by the North
Carolina Division of Environmental Management.
7. Reports on the operations of the facilities shall be submitted to the
Washington Regional Office of the Division of Environmental Management at
regular intervals and in such form and detail as mny be required by this Division.
8. Diversion or bypassing of the untreated wastewater from the treatment facilities
is prohibited.
9. The issuance of this permit shall not relieve the Permittee of the responsibility
for damages to surface or groundwaters resulting from the operation of this
facility.
10. The facilities shall be effectively maintained and operated as a non -discharge
system to prevent the discharge of any wastewater resulting from the operation of
this facility.
11. The Permittee, at least six (6) months prior to the expiration of this permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the facilities described therein, and if warranted, will
extend the Permit for such period of time and under such conditions and
limitations as it may deem appropriate.
12. A staff gauge, for the purpose of determining liquid waste levels, shall be
installed in the waste lagoon.
13. The Permittee shall submit monthly monitoring reports to the Washington Regional
Office which include weekly readings of the staff gauge and the gallons of water
used each month by Washington Packing Company, based on the water readings.
14. All reports required by this permit shall be forwarded to the Washington Regional
Office. Any questions should be referred to the Washington Regional Office

15. The Permittee shall maintain a suitable land application site, properly permitted
by the Division of Environmental Management, for the application of wastewater in
the event that the existing system fails.
