Truck Maintenance Garage Wastewater Treatment and Disposal Facility Catawba County Dear Mr. Garrison: In accordance with your application received November 30, 1988, we are forwarding herewith Permit No. WQ0000864, dated January 23, 1989, to Comm/Scope, Incorporated for the operation of the subject wastewater treatment and disposal system. This permit shall be effective from the date of issuance until April 30, 1991, and shall be subject to the conditions and limitations as specified therein.. This permit shall void Permit No. 16110, issued April 25, 1988, If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this permit shall be final and binding. One (1) set of approved plans and specifications is being forwarded to you. Catawba County FOR THE continued operation of a 600 GPD wastewater treatment and subsurface disposal facility consisting of approximately 160 linear feet of 8 -inch gravity sewer, a 1,000 gallon grease and oil separator, a 30 GPM dosing pump station with dual submersible pumps and high water alarms, 510 linear feet of 4 -inch nitrification line in 3 -foot trenches, and a reserve disposal field area to serve Comm/Scope, Inc. with no lischarge to the surface waters of the State, pursuant to the application received November 30, 1988, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until April 30, 1991, and shall be subject to the following specified conditions and limitations: 1. This permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities shall be properly maintained and operated at all times. 4. This permit is not transferable. 5. A set of approved plans and specifications for the subject project must be retained by the applicant for the life of the project. 6. This permit shall become voidable in, the event of failure of the soil to adequately absorb the wastes and may be rescinded unless the facilities are installed, maintained, and operated in a manner which will protect the assigned water quality standards of the surface waters and ground waters. 7. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or disposal facilities. 8. Any monitoring deemed necessary by the Division of Environmental Management to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 9. Diversion or bypassing of the untreated wastewater from the treatment facility is prohibited. 10. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate. 11. The Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge of the wastewater treatment facilities. Such operator must hold a certificate of the grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. 12_ The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 13. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 14. No type of wastewater other than that from Comm/Scope Truck Maintenance Facilities shall be discharged into the subject facilities. 15. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal maintenance is being performed. 16. An acceptable reserve area, equal in size to the approved disposal area, shall be maintained at all times. 17. Solids, sludges, or other pollutants removed or resulting from the wastewater storage facilities shall be contained and disposed of in such a manner as to prevent any contamination of the surface or groundwaters of the State. 18. Vehicle cleaning operations must entail washing only the outsides of trucks and cars. Oil drained from engine crankcases or solvent degreasing wastes must not be discharged to the disposal system. 19. This permit shall void Permit No. 16110, issued April 25, 1988.
