Dear Mr. Johnson:
In accordance with your application received December 1, 1988, we are
forwarding herewith Permit No. WQ0000855, dated March 23, 1989, to Data General
Corporation for the construction and operation of the subject wastewater treatment
and disposal facilities.
This permit shall be effective from the date of issuance until February 28,
1991, shall void Permit No. 5038122, which was issued on August 26, 1983, and shall
be subject to the conditions and limitations as specified therein.
The Division's Groundwater Section has stated that periodic lead levels in
excess of the groundwater quality standards have been detected in the two oldest
monitor wells. It would be advisable for Data General to investigate the cause of
these high lead levels.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request must
be in the form of a written petition, conforming to Chapter 150E of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this
permit shall be final and binding.

For the discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
continued operation of a 0.20 MGD metal plating wastewater treatment and spray
irrigation facility which consist of a 0.25 million gallon equalization tank, a 73
GPM chelated and non -chelated combined wastewater package treatment unit, including
facilities for cyanide destruction, copper, nickel, and lead precipitation, pH
adjustment, pressure filtration with in-place filter washing, a sludge press, dual 80
GPM pumps, approximately 600 linear feet of 4 -inch force main, a single cell 0.8
million gallon storage pond with a 3.26 CFM aerator, dual 390 GPM pumps, approximately
27.5 acres of spray irrigation area, and all other appurtenances; and for the
construction and operation of approximately 2,225 linear feet of 4 -inch and 6 -inch
force main, approximately 9,100 linear feet of spray irrigation laterals,
approximately ten acres of spray irrigation fields, and all piping, valves, sprinkler
nozzles, and other appurtenances to serve Data General Corporation in Johnston
County, pursuant to the application received December 1, 1988, and in conformity
with the project plan, specifications, and other supporting data subsequently filed
and approved by the Department of Natural Resources and Community Development and
considered a part of this permit.
This permit shall be effective from the date of issuance until February 28, 1991,
shall void Permit No. 5038R2, which was issued on August 26, 1983, and shall be
subject to the following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
3_ The facilities shall be properly maintained and operated at all tames.
4. This permit is not transferable.
5. This permit shall become voidable in the event of failure of the soil to
1 adequately absorb the wastes and may be rescinded unless the facilities are
installed, maintained, and operated in a manner which will protect the assigned
water quality standards of the surface waters and ground waters.
6. In the event that the facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take such immediate
corrective action as may be required by this Division, including the construction
of additional or replacement wastewater treatment or disposal facilities.
7. The Raleigh Regional Office, phone no. 919/733-2314 shall be notified at least
twenty-four (24) hours in advance of operation of the installed facilities so
that an in-place inspection can be made. Such notification to the regional
supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00
p.m. on Monday through Friday, excluding State Holidays.
8. Any monitoring deemed necessary by the Division of Environmental Management to
insure surface and ground water protection will be established and an acceptable
sampling reporting schedule shall be followed_
9. The sludge generated from these treatment facilities must be disposed of in
accordance with General Statute 143-215.1 and in a manner approvable by the North
Carolina Division. of Environmental Management.
10. Satisfactory controls shall be provided to contain the accidental discharge or
spillage of hazardous or toxic substances either stored or being used on the
project site.
11. A suitable year round vegetative cover shall be maintained.
12. Irrigation shall not be performed during inclement weather or when the ground is
in a condition that will cause runoff.
13. Adequate measures shall be taken to prevent wastewater runoff from the spray
field.
14. Diversion or bypassing of the untreated wastewater from the treatment facility
is prohibited.
15. The Permittee, at least six (6) months prior to the expiration of this permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the facilities described therein, and if warranted, will
extend the Permit for such period of time and under such conditions and
limitations as it may deem appropriate.
16. The application rates shall not exceed 0.4 inches per hour and 2 inches per week.
17. The Permittee shall employ a certified wastewater treatment plant operator to be
in responsible charge of the wastewater treatment facilities. Such operator must
hold a certificate of the grade at least equivalent to the classification
assigned to the wastewater treatment facilities by the Certification Commission..
18. Freeboard in the lagoon shall not be less than two feet at any time.
19. The issuance of this permit shall not relieve the Permittee of the responsibility
for damages to surface or groundwaters resulting from the operation of this
facility.
20. No type of wastewater other than that from Data General Corporation shall be
sprayed onto the irrigation area.
21. No traffic or equipment shall be allowed on the disposal area except while
installation occurs or while normal maintenance is being performed.
22. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee
to insure proper operation of the subject facilities.
23. In any future transfer of this land, a notice shall be given to the new owner
that gives full details of the materials applied or incorporated at this site.
24. A set of approved plans and specifications for the subject project must be
retained by the applicant for the life of the project.
25. The following buffer distances shall be maintained between the spray irrigation
field:
a. 400 ft. to residences
b. 100 ft. to streams, lakes, etc.
C. 100 ft. to drinking water supplies (wells)
d. 150 ft. to property lines
e. 50 ft. to public right-of-ways
26. Warning signs shall be placed around the spray irrigation fields.
27. The highly disturbed soils in fields "N" and "P" must be thoroughly reworked and
replanted prior to any application of wastewater.
28. The permittee should maintain records which include, but shall not necessarily be
limited to the following:
a. date and time of spray irrigation
b. field(s) wastewater applied to
C. volume of wastewater applied
d. weather conditions at time of irrigation
e. cummulative and year to date loadings of organics and metals
This information shall be submitted to the Raleigh Regional Office around August
lst of each year.
29. The results of an annual soil. (of irrigation site) evaluation and wastewater
analysis shall be submitted to the Raleigh Regional Office around August 1st. of
each year.
30. Prior to beginning waste disposal operations at the new spray fields, three (3)
monitor wells, one (1) upgradient and two (2) downgradient, shall be installed to
monitor the quality of the "GA" classified groundwater. The location and
construction details for these wells shall be approved by the Raleigh Regional
Office from which a well construction permit must be obtained. (Note: The well
locations and construction details approved by the Division may be different from
the well locations and construction details indicated on the approved plans).
31. The three (3) new monitor wells shall be sampled initially after construction
(and prior to waste disposal operations) and thereafter every March, July, and
November for the following parameters:
TDS pH
Chloride Chromium
Copper Lead
TOX (in Nov. only) Water Level*
zinc Cadmium
The measurement of water level must be made prior to sampling for the remaining
parameters.
The four (4) existing monitor wells shall. also be sampled every March, July and
November for the parameters specified above.
The analytical methods used for TOX must be capable of detecting the total of all
halogenated organic compounds present at a concentration of 5.0 parts per billion
(ppb) or greater. In the event that a TOX concentration of 5.0 ppb or greater is
detected, any individual halogenated organic compound(s) present at a
concentration. at or above the method detection l=imit (MDL) must be identified and
quantified utilizing EPA methods 601, 602, 601+ and 611.
The results of the sampling and analysis shall be sent to the N.C. Division of
Environmental Management on Form GW -59 (Compliance Monitoring Report Form) every
April, August and December_

the penalty provisions applicable under General Statute
143-215.6(1)a.
The sale of property by the Permittee, which
is within

contiguous to
the disposal, system site, may alter, the location of
the Perimeter.

32. Adequate storage facilities shall be provided for waste sludge.
33. Hazardous sludge shall be disposed as directed by the Department of. Human
Resources - Division of Health Services,
34. The soi.1 pH of the irrigation areas shall be maantai.ned at or above 6.5. The pH
of the wastewater in the storage .Lagoon shall be 6-9 standard units.
35. if the capacity of the existing storage pond proves to be inadequate for holding
} the necessary volume of wastewater, due to prolonged adverse weather conditions,
Data General shall reduce the wastewater discharge flowrate or shall cease
wastewater discharge altogether, until conditions improve to the point that
irrigation can resume and adequate storage is available.
