Dear Mr. Smith:
In accordance with your permit modification application package received on December 13,
2006, as well as the additional information received on February 8, 2007; we are forwarding herewith a
modified Attachment B, certified February 15, 2007, to the Granville Farms, Inc. for the subject residuals
land application program.
This certification is being issued to modify Attachment B to:
♦ Approve new land application sites owned by Jim Moss in Franklin County and Clifton Moss
in Vance County for land application of residuals under this program. These sites total
144.36 net acres.
Please replace the existing Attachment B of Permit No. WQ0000838 that was previously
certified on August 15, 2006, with this modified Attachment B. Note that only the vicinity and buffer
maps for any new and modified land application sites are attached to this modified Attachment B. It is
the Permittee's responsibility to ensure that a complete permit is created by inserting this modified
Attachment B and vicinity and buffer maps into Permit No. WQ0000838, issued on August 15, 2006, and
making sure to retain any vicinity and buffer maps for any existing and unmodified land application sites.
.This modified Attachment B shall be effective from the date of certification; shall void
Attachment B of Permit No. WQ0000838 that was previously certified on August 15, 2006; and shall be
subject to the conditions and limitations as specified therein and well as those specified in the above -
referenced permit. As always, remember to take the time to review this modified Attachment B
thoroughly, as some of the conditions contained therein may have been changed since the last
certification. Note that this modified Attachment B is hereby incorporated by reference into Permit No.
WQ0000838, issued on August 15, 2006.

Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the from of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.

Groundwater monitoring wells MW -1, MW -2, MW -3, and MW -4 located at this land application site shall be maintained; however, sampling shall not be required.
The wells shall have their wellheads secured in a way such as to prevent accidental damage or access by hand. This access restriction shall be accomplished
by utillizing a padlock that would secure a protective well cap casing or by some other means acceptable to the Division's Aquifer Protection Section.

Granville County
The Division of Water Quality's (Division) Raleigh Regional Office has completed a thorough
technical review of the subject permit inodification application package. At this time, additional
information is required before we may continue our review. Please address the items listed below no
later than February 12, 2007.
Buffer Maps:
1. General: The 400 -foot buffers for houses are on the maps, however the houses themselves
are not labeled. All houses within 500 feet of the application area need to be labeled. All
wells within 500 feet of the application area also need to be added to the map.
2. Field CM -12 (Clifton Moss Site): A new house has been constructed north of Field 12. This
house needs to be added to the reap, and given a 400 -foot buffer.
3. Field CM -13: A wet area to the south of the small buffered area already on the snap needs to
be added and given a buffer. The buffered area on the south side of the field appears to be
usable. If so desired, you can add this area back onto the map.
4. Unnamed Field: The field in between Fields CM -7 and CM -8 appears to have some usable
area and can be added to the application. There is an area of excessive slope and a stream on
the south side of this field that will need to be buffered out. In addition, a soil report for this
field will need to be submitted.
Field JM -7 (Jimmy Moss Site): There is a small area at the entrance to the field on the path
between fields 6 and 7 (the southeast corner) that needs to be buffered due to a ditch between
the two fields.

6. Field JM -9: This field is being removed from the application'due to the small size.
7. Field JM -10:
a. Two houses at the corner of Hicks Road and Cedar Creek Road need to be added to
the buffer map with 400 -foot buffers. One is located at 2427 Cedar Creek Road, and
the other is at 2468 Cedar Creek Road. Depending on the remaining acreage this
field may be deleted from the application.
b. The Franklin County GIS website shows that the 2 acre portion of this field is owned
by Mattie Frazier Woodlie£ Please confirm ownership of this field, and if necessary,
re -submit the landowner agreement.
8. . Field JM -11:
a. Depending on the status of Field JM -10, this field maybe removed from the
application.
b. The Franklin County GIS website shows that this field is owned by the estate of Ruby
E. Conyers. Please confirm ownership of this field, and if necessary, re -submit the
landowner agreement.
