In accordance with your permit modification application package received on October 9, 2002 as
well as the additional information received on November 7, 2002 and December 17, 2002; we are
forwarding herewith a modified Attachment B, certified January 17, 2003, to Granville Farms, Inc. for the
subject residuals land application program.
This certification is being issued to approve a number of changes in the identification number,
acreage, and applicable groundwater restrictions to the land application sites owned by Granville Farms,
Inc. in Granville County following the receipt of updated buffer maps and data associated with your
request- Of particular interest may be the following issues:
♦ The total combined acreage permitted in these sites has decreased from 1,235 acres in the
previous issuance of Attachment B to 515.9 acres in this issuance.
♦ Site GF -007 has been split into two sites (GF -007 and GF -007A).
♦ The seasonal restrictions on Sites GF -001, GF -002, GF -003, GF -004, GF -006, GF -007A, GF -
008, GF -009, GF -010 and GF -012) have changed to allow for year round application with
verification of water table depth through borings between the months of April and November,
inclusive.
♦ A number of footnotes have been deleted or modified following the completion of required
conditions associated with those footnotes.
Please replace the existing Attachment B of Permit No. WQ0000838 that was previously certified
on September 13, 2002, with this modified Attachment B. Note that only the vicinity and buffer maps for
any new and modified land application sites are attached to this modified Attachment B. It is the
Perriittee's responsibility to ensure that a complete permit is created by inserting this modified
Attachment B and vicinity and buffer maps into Permit No. WQ0000838, issued on September 13, 2002,
and making sure to retain any vicinity and buffer maps for any existing and unmodified land application
sites.

This modified Attachment B shall be effective from the date of certification; shall void
Attachment B of Permit No. WQ0000838 that was previously certified on September 13, 2002; and shall
be subject to the conditions and limitations as specified therein and well as those specified in the above -
referenced permit. As always, remember to take the time to review this modified Attachment B
thoroughly, as some of the conditions contained therein may have been changed since the last
certification. Note that this modified Attachment B is hereby incorporated by reference into Permit No.
WQ0000838, issued on September 13, 2002.
If any parts, requirements, and/or limitations contained in this certification or modified
Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the from of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.

Only the residuals generated by the specified residuals source -generating facility(is) identified are approved for land application on the specified land application site.
n When water treatment plant residuals are land applied to this land application site, wastewater treatment plant residuals shall also be land applied to the site during the same crop season
to supplement phosphorus.
Within 60 calendar days of this permit's issuance, the inactive water supply well located on this land applications site shall be permanently abandoned. The well shall be abandoned
by either a North Carolina -certified well contractor, the property owner, or the property lessee according to North Carolina General Statutes 87-98.4(b)(2). If the abandonment
is not performed by a North Carolina -certified well contractor, the property owner or lessee shall physically perform the actual well abandonment activities and the well must be abandoned
according to 15A NCAC 2C.01 13 (i.e., "Abandonment of Wells") and any applicable local county rules. Within 30 calendar days of the well's abandonment, Form GW -30
i.e., "Well Abandonment Record") shall be completed for the well abandoned and mailed to the NCDENR-DWQ, Groundwater Section, Permits and Compliance Unit,
1636 Mail Service Center, Raleigh, NC 27699-1636.
No residuals shall be applied to this land application site when the vertical separation between the depth of application and the water table is less than three feet. Between the months of April
through November (inclusive). The actual water table depth must be verified by soil borings, prior to any land application event. The number of borings advanced shall be sufficient to adequately
characterize water table conditions across the entire proposed application site. Any open borings must be properly filled with native soil, prior to application, to decrease the chances of any
residuals contaminating groundwater.
Groundwater monitoring wells MW -1, MW -2, MW -3, and MW4 located at this land application site shall be maintained; however, sampling shall not be required. The wells shall have
their wellheads secured in a way such as to prevent accidental damage or access by hand. This access restriction shall be accomplished by utilizing a padlock that would secure
a protective well cap to the protective casing or by some other means acceptable to the Division's Groundwater Section.
F The Compliance Boundary for this land application site shall extend into the site and around the water supply well, maintaining a radial distance of 150 feet from the well.
a This land application site is partially covered in soils having a seasonal high water table at depths ranging from zero to three feet below the land surface. Any area found to have
a seasonal high water table at a depth of less than one foot below land surface shall be buffered from residuals land application at all times.
