In accordance with your permit modification application package received on May 16, 2003 as well
as the additional information received on June 5, 2003; we are forwarding herewith a modified Attachment A,
certified June 27, 2003, to Granville Farms, Inc. for the subject residuals land application program.
This certification is being issued to modify Attachment A to approve residuals from six additional
residuals source -generating facilities (i.e., package -plant type) for land application under this program. These
facilities, which are all located in Wake County and will contribute a maximum of 69.50 dry tons per year to
the volume of residuals allowed to be land applied, are as follows:

Wallace Woodall Vacuum Pumping Services, Inc., which is currently in the process of being rescinded by the
Division of Water Quality (Division).

As always, remember to take the time to review this modified Attachment A thoroughly, as some
of the conditions contained therein may have been changed since the last certification. Of particular interest
to you may be the following:
Footnote a states that only residuals from the residuals source -generating facility treating Shearon
Harris Power Plant's domestic wastewater (i.e., serves employees only) shall be approved for
land application. A permit modification application package must be submitted and approved by
the Division should it be desired that residuals from any other facility, including the Harris
Energy & Environmental Center, at Shearon Harris Power Plant be approved for land application
at a future point in time.
Analytical results for several parameters required for permitting were not provided in the
submitted permit modification application package or the results as listed in the package were
highly suspect. In consideration of the immediate need for residuals to be hauled from these
residuals source -generating facilities, the Division has decided to go forward with this
certification. However, note that Footnotes d and f require that certain analyses be performed and
the results submitted to the Division prior to initiating a land application event from the
referenced facility. The receipt of the results must be acknowledged by the Division in writing
before the Permittee may proceed with the event.
Information presented in the permit modification application package states that the residuals
generated at Pope Industrial Park are domestic in nature. Therefore, this residuals source -
generating facility has been exempted from the hazardous waste characteristics monitoring
required by Condition III. 2. of the permit. Note, however, that Footnote g requires that this
monitoring be performed on an annual frequency at any time that industrial wastewater is
introduced to the facility.
Please replace the existing Attachment A of Permit No. W00000838 that was previously
certified on September 13, 2002, with this modified Attachment A. Note that only the vicinity maps for
any new and modified residuals source -generating facilities are attached to this modified Attachment A. It is
the Permittee's responsibility to ensure that a complete permit is created by inserting this modified
Attachment A and vicinity maps into Permit No. W00000838, issued on September 13, 2002, and making
sure to retain any vicinity maps for any existing and unmodified residuals source -generating facilities.
This modified Attachment A shall be effective from the date of certification; shall void Attachment A
of Permit No. W00000838 that was previously certified on September 13, 2002; and shall be subject to the
conditions and limitations as specified therein and well as those specified in the above -referenced permit. As
always, remember to take the time to review this modified Attachment A thoroughly, as some of the
conditions contained therein may have been changed since the last certification. Note that this modified
Attachment A is hereby incorporated by reference into Permit No. W00000838, issued on September 13,
2002.
If any parts, requirements, and/or limitations contained in this certification or modified Attachment A
are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days
following receipt of the certification. This request shall be in the from of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at
6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such demands are made, this permit
shall be final and binding.
