In accordance with written (emailed) request received on November 8, 2004, we are forwarding herewith the
corrected Cover Letter and page I of Permit No. WQ0000819, issued October 14, 2004, for the subject
facility.
The following corrections have been made to the permit:
1. The address has been corrected on the Cover Letter.
2. The system description on Page one (I) of the permit has been changed to indicate a liquid
feed chlorination system is now being used.
Please replace the Cover Letter with the corrected Cover Letter and page one with the corrected page
one into Permit No. WQ000000819, issued October 14, 2004.
