Box 33189 Charlotte, N.C. 28242 SUBJECT: Permit No. WQ0000788 Duke Power Company Winston-Salem Northern Satellite Facility Oil Seperator/Drain Field Forsyth County Dear Mr. Biddle: In accordance with your application received November 14, 1988, we are forwarding herewith Permit No- WQ0000788, dated May 3, 1989, to Duke Power Company for the construction and operation of the subject wastewater collection system extensions. This permit shall be effective from the date of issuance until April 30, 1994, and shall be subject to the conditions and limitations as specified therein_ If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This .request roust be in the form of a written petition, conforming to Chapter 150E of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this permit shall be final and binding. One (1) set of approved plans and specifications is being forwarded to you. Box 27687, Raleigh, North Carolina 27611-7687 Telephone 919-733-7015 NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT RALEIGH P E R M I T For the discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 1.43, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO Duke Power Company Forsyth County FOR THE construction and operation of a 720 GPD non -discharge wastewater treatment system consisting of an oil/water separator, a 20 GPM pump station with dual submersible pumps and high water alarms, approximately 350 linear feet of 2 -inch force main, a distribution box, 1800 linear feet of drain line, 9,375 square feet of reserve area and all associated valves, pipes and appurtenances to serve Duke Power Company's Northern Satellite Garage in Forsyth County, pursuant to the application received November 14, 1988, and in conformity with the project plan., specifications, and other supporting data subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until April 30, 1994, and shall be subject to the following specified conditions and limitations: I. This permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities shall be properly maintained and operated at all times. 4. This permit is not transferable. 5. Construction of the facility shall be so scheduled so as not to interrupt service by the existing utilities nor result in an overflow or bypass discharge of wastewater to the surface waters of the State. 6. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the requirements of this permit and the approved plans and specifications. Mail the certification to the Permits and Engineering Unit, P.O. Box 27687, Raleigh, N.C_ 27611-7687. 7. A set of approved plans and specifications for the subject project must be retained by the applicant for five (5) years after completion of construction. 8. This permit shall become voidable in the event of failure of the soil to Adequately absorb the wastes and may be rescinded unless the facilities are installed, maintained, and operated in a manner which will protect the assigned water quality standards of the surface waters and ground waters. 9. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or disposal facilities. 10. Monitoring wells shall be constructed and sampling schedules arranged as required by the Division of Environmental Management and in accordance with 15 NCAC 2C .0100. 1.1. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate. 12. Any monitoring deemed necessary by the Division of Environmental Management to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 13. An acceptable reserve area, equal in size to the approved disposal area, shall be maintained at all times. 14. Any additional groundwater quality monitoring as deemed necessary by the Department, shall be provided. 15. The subsurface disposal field trenches shall be installed on contour with the site topography. 16. The reserved area shall be maintained as undisturbed soils. 17. Prior to beginning waste disposal operations, two (2) monitor wells, one (1) upgradient and (1) one downgradient, shall be by approval of the Winston-Salem Regional Office, from which a well construction permit must be obtained. 18. The monitor wells shall be sampled initially after construction (and prior to waste disposal operations) and thereafter every November for the following parameters: PH Purgeable Halocarbons & Purgeable Aromatics (as per EPA methods 601 and 602) MBAS TDS Water Level Lead The measurement of water level must be made prior to sampling for the remaining parameters. The results of the sampling and analysis shall be sent to the N.C. Division of Environmental Management on Form GW -59 [Compliance Monitoring Report Forml every December. 19. No type of oil, gasoline, solvents or antifreeze shall be discharged to the nitrification field. As outlined in the permit application, all used motor oil shall be collected and disposed of by an approved waste oil vendor. Used engine coolant will be collected and recycled with waste by-products which are being disposed of in an approved manner off-site. Permit issued this the 3rd day of May, 1989.
