S&B Maintenance Residuals Land Application Program (D) Land Application of Residual Solids (503) Brunswick County In accordance with your permit modification application package received on June 5, 2007, we are forwarding herewith a modified Attachment A, certified August 14, 2007, to S&B Maintenance, Inc. for the subject residuals land application program. This certification is being issued to modify Attachment A by the addition of one new residuals source -generating facility (West Brunswick Regional WWTF, the deletion of nine previously permitted residuals source -generating facilities (Brunswick Community College WWTP, Hood Creek (Northwest), Elementis Chromium, L.P. Castle Hayne, NC Facility, Ocean Isle Beach SBR WWTF, Beau Rivage Plantation WWTF, Dolphin Bay WWTP, The Cape WWTP, Winding River Plantation WWTF, and Belville WWTP), the adjusting of maximum dry tons per year from 1,582.14 to 847.34. The Attachment A is also being modified to reflect the owner/facility's name changes. Please replace the existing Attachment A of Permit No. WQ0000783 that was previously certified on December 8, 2005, with this modified Attachment A. Note that only the vicinity map for any new residuals source -generating facilities are attached to this modified Attachment A. It is the Permittee's responsibility to ensure that a complete permit is created by inserting this modified Attachment A and vicinity maps into Permit No. WQ0000783, issued on December 8, 2005, and making sure to retain any vicinity maps for any existing residuals source -generating facilities. This modified Attachment A shall be effective from the date of certification; shall void Attachment A of Permit No. WQ0000783 that was previously certified on December 8, 2005; and shall be subject to the conditions and limitations as specified therein and well as those specified in the above - referenced pen -nit. As always, remember to take the time to review this modified Attachment A thoroughly, as some of the conditions contained therein may have been changed since the last certification. Note that this modified Attachment A is hereby incorporated by reference into Permit No. WQ0000783, issued on December 8, 2005. If any parts, requirements, and/or limitations contained in this certification or modified Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with. the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Note that a permit modification application package shall be submitted and approved by the Division should it be desired that residuals from any other facility at this location be approved for land application at a future point in time.
