Box 70 Lake Toxaway, NC 28747 SUBJECT: Permit No. WQ0000731 Lake Toxaway Company Wastewater Collection, Treatment and Golf Course Spray Irrigation Disposal Facilities Transylvania County Dear Mr. Johnson: In accordance with your application received November 2, 1988, we are forwarding herewith Permit No. WQ0000731, dated January 30, 1989, to Lake Toxaway Company for the construction and operation of the subject wastewater collection, treatment and disposal facilities. This permit shall be effective from the date of issuance until January 31. 1994, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 1.1666, Raleigh, North Carolina 27604. Unless such demands are made this permit shall be final and binding. One (1) set of approved plans and specifications is being forwarded to you. Box 27687, Raleigh, Noreh Carolina 27611-7687 Telephone 919-733-7015 An Equal opportunity- Affirmative Action Employer NORTH CAROLINA ENVIRONMENTAI, MANAGEMENT COMPASSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT RALEIGH P E R M I T For the discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO Lake Toxaway Company Transylvania County FOR THE construction and operation of 20,000 GPD wastewater_ collection, treatment and golf coarse spray irrigation disposal facility. The collection system consisting of septic tanks, a 15 GPM duplex, grinder, influent pump station with high water alarms and 2,750 linear feet of 1 1/2 -inch force main. The treatment facilities consisting of a 5,000 gallon aerated surgo tank, dual 20,00.0 gallon aeration tanks, dual 3,395 gallon clarifiers, dual 1,680 gallon sludge holding tanks, a total of four (4) 74 CFM blowers, a dual cell tertiary filter, a tablet chlorinator, 520 gallon chlorine contact tank and stand-by power. The golf course spray irrigation disposal facilities consisting of a 1.00,000 gallon 5 -day detention pond, a 150,000 gallon irrigation pond, a 400 GPM effluent irrigation pump station, 1,800 linear feet of 6 -inch force main and approximately four (4) acres of spray irrigation a.rea on fairways Nos. 3, 4, 6, and 7 at hake Toxaway with no discharge to the surface water of. the State. pursuant to the application received November 2, 1988, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Natural. Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until January 31, 1994, and shall be subject to the following specified conditions and limitations: 1. This permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities shall be properly maintained and operated at all times. 4_ The sewage and wastewater collected by this system shall be treated in the hake Toxaway Wastewater Treatment Plant prior to being spray irrigated. 5. This permit is not transferable. 6. Construction of the sewers shall be so scheduled so as not to interrupt service by the existing utilities nor result in an overflow or bypass discharge of wastewater to the surface waters of the State. 7. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the requirements of this permit and the approved plans and specifications. Mail the certification to the Permits and Engineering Unit, P.O. Box 27687, Raleigh, N.C. 27611-7687. 8. A set of approved plans and specifications for the subject project must be retained by the applicant for the life of the project. 9. This permit shall become voidable in the event of failure of the soil to adequately absorb the wastes and may be rescinded unless the facilities are installed, maintained, and operated in a manner which will protect the assigned water duality standards of the surface waters and ground waters. 10, In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or disposal facilities. 11. The Asheville Regional Office, phone no. (704) 251--6208 shall be notified at least twenty-four (24) hours in advance of operation of the installed facilities so that an in-place inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. 12. The sludge generated from these treatment facilities must be disposed of in accordance with General Statute 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. 13. The subject facilities shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters which will render them unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions or failure of the irrigation area to adequately absorb the wastewater, the Permittee shall take such immediate corrective action as may be required by the Division of Environmental Management. 14. A suitable year round vegetative cover shall be maintained. 15. Irrigation shall not be performed during inclement weather or when the ground is V n a condition that will cause runoff. 16. Adequate measures shall be taken to prevent wastewater runoff from the spray field. 17. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. 18. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate. 19, The application rates shall not exceed .4 gallons per square foot. 20. The Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge of the wastewater treatment facilities. Such operator must hold a certificate of the grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. 21. The Certified Operator shall be on call. twenty-four (24) hours per day, 22. Freeboard in the detention and irrigation ponds shall not be less than two (2) feet at any time. 23, The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility, 24. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this .facility. 25. No type of wastewater other than that from bake Toxaway shall be sprayed onto the irrigation area. 26. The Permittee shall provide for the pump station and .force main the following items: a. Pump-on/pump-off elevations located so that 2-8 pumping cycles may be achieved per hoar in the pump station. b. An air relief valve located at all high points along the force main. C. A screened vent for the wet well. d. Fillets located in the wet well at the intersection of the flooring and sidewalls. e. Three (3) feet of cover (minimum) over the force main or the use of ferrous material where three (3) feet cannot be maintained. f. Sufficient devices which will protect the pump station from vandals. g. Flood protection if the pump station is located below the 100 -year flood elevation. 27. Solids, sludges, or other pollutants removed or resulting from the wastewater storage facilities shall be contained and disposed of in such a manner as to prevent any contamination of the surface or groundwaters of the State. 28. The treatment process shall produce an effluent with a monthly average TSS of less than 5 mg/l and a daily maximum TSS of less the 10 mg/l and a maximum fecal coliform level of less than 1/100 ml, prior to discharge to a five-day detention pond. .29. There must be no public access to the five-day detention pond. 30. In the design of the sprinkler system, the piping shall. be a separate system, with no cross -connections to a potable water supply (includes no spigots on the distrubition system). 31. The time of spraying shall occur between 11:00 p.m. and three hours prior to the daily opening of the course. 32. There shall be a 100 foot vegetative buffer zone between the edge of spray influence and the nearest dwelling. 33. Signs shall be posted at the proshop stating that the course is irrigated with treated wastewater, 34. Prior to beginning waste disposal operations, five (5) monitor wells, one (1) upgradient and (1) downgradient of the lagoons and one (3.) upgradient and two (2) downgradient of the irrigation site(s), shall be installed to monitor groundwater quality. The location and construction details for these wells shall be approved by the Asheville Regional Office, from which a well construction permit must be obtained. 35. The monitor wells shall be sampled i_nitally after construction (and prior to waste disposal operations) and thereafter every March, July, and November for the following parameters: NO3 Ammonia Nitrogen TDP Total Coliform TOC pR Chloride TOX (in November only) Water Level. The measurement of water level must be made prior to sampling for the remaining parameters. If TOC concentrations greater than 10 mg/l. are detected in any downgradient monitoring well, additional sampling and analysis must be conducted to identify the individual constituents comprising this TOC concentration. If the TOC concentration as measured in the background monitor well exceeds 10 mg/l, this concentration will be taken to represent the naturally occuring TOC concentration_ Any exceedances of this naturally occurig TOG concentration in the downgradient wells shall be subject to the additional sampling and analysis as described above. The analytical methods used for TOX must be capable of detecting the total of all halogenated organic compounds present at a concentration of 5.0 parts per billion (ppb) or greater. In the event that a TOX concentration of 5.0 ppb or greater is detected, any individual halogenated organic compound(s) present at a concentration at or above the method detection limit (MDL) must be identified and quantified utilizing EPA methods 601, 602, 604, and 611. The results of the sampling and analysis shall be sent to the NC division of Environmental Management of Form GW -59 [Compliance Monitoring Report Form) every April, August, and December, 36. The interior surfaces of both the detention lagoon and the irrigation lagoon shall be completely lined with a 40 -mil thick Hypalon liner. following installation and inspection of the Hypalon lagoon liner, and prior to waste disposal operations, certification of the liner's compliance with approved construction specifications and the liner's integrity must be provided to the Division of Environmental Management, Groundwater Section, by the project engineer. 37. Any additional groundwater quality monitoring, as deemed necessary by the Division, shall be provided. 38. Wastewater shall not be land applied with 100 ft. of a water supply well.
