Beacon's Reach Development Wastewater Treatment and Disposal Facilities Carteret County Dear Mr. Fulcher: In accordance with your application received October 31, 1988, we are forwarding herewith Permit No. WQ0000703, dated December 30, 1988, to WPKS Utilities, Inc. for the construction and operation of the subject wastewater treatment and disposal facilities. This permit shall be effective from the date of issuance until December 31, 1.993, shall void Permit No. 59078, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 1.50B of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this permit shall be final and binding. One (1) set of approved plans and specifications is being forwarded to you. Carteret County FOR THE construction and operation of additional wastewater treatment facilities consisting of a 35,000 GPD extended aeration package plant with clarification, tertiary filters with 24 square feet surface area, a tablet chlorinator, a 729 gallon chlorine contact chamber, an effluent dosing chamber with dual 84 GPM submersible pumps and approximately 3,910 linear feet of 3 feet wide disposal trench. Existing treatment facilities consist of a 60,000 GPD extended aeration package plant with clarification, two equalization basins (60,000 gallons capacity), a 20,000 gallon aerobic digestor, tertiary filters with 21 square feet surface area, a tablet chlorinator, 1,250 gallon chlorine contact chamber, an effluent dosing chamber, and approximately 12,020 linear feet of 3 feet wide disposal trench; and a. 35,000 GPD "Defiance" package plant with tertiary filters, disinfection chamber, and approximately 9,000 linear feet of 3 feet wide disposal trench; and disposal of sludge by land application via Beach Disposal Service (Permit No. 9091AIR) to serve Beacon's Reach Development, Ramada Inn, and Maritime West with 130,000 GPD ultimate capacity, pursuant to the application received October 31, 1988, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until December 31, 1993, and shall be subject to the following specified conditions and limitations: 1. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the approved plans and specifications. Mail the certification to the Permits and Engineering Unit, PO Box 27687, Raleigh, NC 27611-7687. 2. This permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. 3. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 4. The facilities shall be properly maintained and operated at all times. 5. This permit is not transferable. 6. This permit shall become voidable in the event of failure of the soil to adequately absorb the wastes and may be rescinded unless the facilities are installed, maintained, and operated in a manner which will protect the assigned water quality standards of the surface waters and ground waters. 7. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or, replacement wastewater treatment or disposal facilities. 8. The Wilmington Regional Office, phone no. 919/256-4161 shall be notified at least twenty-four (24) hours in advance of operation of the installed facilities so that an in-place inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State holidays. 9. Any monitoring deemed necessary by the Division of Environmental Management to Insure surface and ground water protection will be established and an acceptable sampling reporting schedule, shall, be followed. 10. The sludge generated from these treatment facii_it:;es must be disposed of in accordance with General Statute 143-215.1 and in a. moaner approvable by the North Carolina Division of Environmental Management. 11. Diversion or, bypassing of the untreated wastewater from the treatment facilities is prohibited. 12. The Permittee, at least six (6) months prior to the expiration of this permit, shall request: its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate. 13. The application rates shall not exceed 3.0 gpd/ft2 14. The Permittee shall. employ a certified wastewater treatment plant operator to be in responsible charge of the wastewater treatment facilities. Such operator must hold a certificate of the grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. 15. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 16. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 17. No type of wastewater other than that from Beacon's Reach Development, Maritime West and the Ramada Inn shall be disposed by subsurface disposal. 18. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal maintenance is being performed. 19. The existing and proposed disposal fields shall be fenced and, posted to prevent access by vehicular traffic. 20. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 21. An acceptable reserve area, equal in size to the approved disposal area, shall be maintained at all times. 22. The sand media of the tertiary filters must comply with the Division's specifications. The engineer's certification will serve as proof of this compliance. 23. A copy of the approved plans and specifications shall be maintained on file for the life of the Permit. 24. Any additional groundwater quality monitoring as deemed necessary by the Department, shall be provided. 25. Standby power shall be provided within 90 days of the issuance of this permit. The Permittee shall notify the Wilmington Regional Office in writing when the standby power is in place and operational. The standby power unit shall be tested monthly. 26. All low pressure lines shall be purged and the pressure equalized on the lilies at least bimonthly. 27. Magnesium annodes shall be installed in all metal structures for corrosion, protection. 28. The rusted areas of the metal structures shall be sanded and spot painted. 29. The existing air diffuser pipes need maintenance. They should be repaired/replaced within 30 days of the issuance of this permit. 30. The air scour on the tertiary filters for the 0.060 MGD plant must be repaired and returned to service within 30 days of the issuance of this permit. 31. A maintenance/ operations log shall be maintained by the operator in responsible charge. The log shall include at a minimum the following information: - Results of any in -plant testing. - bate low pressure lines were cleaned and pressure was adjusted in disposal fields. Date blowers were lubricated, belts replaced, and filters changed. - Date of any plant upset and actions taken to remedy upset condition. Date any sludge is removed from the facility, including hauler, and final disposal site. 32. The Permittee shall conduct a self-monitoring program in accordance with the following schedule: Tests and Measurements Location of Sampling Points Measurement Freauen� Residual. This permit shall become voidable unless the agreement between Beach Disposal Service, Inc. and WPKS Utilities, Inc. for removal and disposal of sludge is in full force and effect.
