In accordance with your permit modification application package received on April 16, 2008 we
�^ are forwarding herewith a modified Attachment B, certified May 5, 2008, to the City of
Havelock for the subject residuals land application program.

This certification is being issued to modify Attachment B by the addition of 132.9 acres located
near Grantsboro in Pamlico County. The additional application area consists of 19 fields,
designated as site NC -PA -2; fields IA to IG, 2A to 2E, and 3A to 3G. Please note that the
footnote regarding the Compliance Boundary for the new fields was removed. The permit
defines the Compliance Boundary as 250 feet from the land application area or 50 feet within the
property line, whichever is closer to the land application area (with exceptions noted in footnote

Please replace the existing Attachment B of Permit No. WQ0000702 that was previously
certified on April 7, 2008, with this modified Attachment B. Note that only the Attachment B
(table of certified fields) is attached to this modified Attachment B. It is the Permlttee's
responsibility to ensure that a complete permit is created by inserting this modified Attachment
B into Permit No. WQ0000702, issued April 7, 2008, and making sure to retain any vicinity and
buffer maps for any existing and unmodified land application sites.

This modified Attachment B shall be effective from the date of certification; shall void
Attachment B's of Permit No. WQ0000702 that were previously certified on April 7, and May 5,
2008; and shall be subject to the conditions and limitations as specified therein as well as those
specified in the above -referenced permit. As always, remember to take the time to review this
modified Attachment B thoroughly, as some of the conditions contained therein may have
been changed since the last certification. Note that this modified Attachment B is hereby
incorporated by reference into Permit No. WQ0000702, issued on April 7, 2008.
If any parts, requirements, and/or limitations contained in this certification or modified
Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of the certification. This request shall be in the
form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes
and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, NC
27699-6714. Unless such demands are made, this permit shall be final and binding. If you

Any field identified with a GW -B classification in prior Land Application of Residual Solids Permit prohibiting laud application events from December through March, inclusive, and when the vertical separation between the
depth of residuals land application and the water table is leas than three feet. 15A NCAC 2T .1109 Operation and Management Practices (6X1)(1) specifies that application of bulk residuals is prohibited when the vertical
separation between the seasonal high water table and the depth of residuals application is less than one foot. By issaance of this modified Attachment B, the GW -B classification and seasonal restrictions are removed from the
noted field. Vertical separation requirements specked in I SA NCAC 2T .1109 must be satisfied at the time of any residuals application event -one foot of vertical separation must be present
A m-dafined Compliance Boundary has been approved for this land application site. This re-dofwed Compliance Boundary is between fields NC-CV-1-lA and NC -CV -1-1C of the field and shall be located at 50 feet from the
water supply well on Robert G. Campbell's property. However, the 100 -foot setback between wells and the land application area must still be met
