Cullinane: In accordance with your application received October 27, 1988, we are forwarding herewith Permit No. WQ0000679, dated November 9, 1988, to the County of Durham for the construction and operation of the subject wastewater collection system extensions. This permit shall be effective from the date of Issuance until rescinded, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request au adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this permit shall. be final and binding. One (1) set of approved plans and specifications is being forwarded to you. Box 27687, Raleigh, North Carolina 27611.7687 Telephone 919-733-7015 An Equal Opportunity Affirmative Action Employer NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT RALEIGH P E R M I T For the discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO County of Durham Durham County FOR THE construction and operation of approximately 1,310 linear feet of 8 --inch and 535 linear feet of 10 -inch gravity sewers to serve lot numbers 1 thru 5 and 52 thru 57 inclusive of Pagehurst Subdivision, Phase 1 and the discharge of 3,960 GPD of collected domestic wastewater into the County of Durham's existing sewerage system, pursuant to the application received October 27, 1988, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until rescinded, and shall be subject to the following specified conditions and limitations: 1. This permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities shall be properly maintained and operated at all times. 4. The sewage and wastewater collected by this system shall be treated in the County of Durham's Triangle Wastewater Treatment Plant prior to being discharged into the receiving stream. 5. This permit is not transferable. 6. Construction of the sewers shall be so scheduled so as not to interrupt service by the existing utilities nor result in an overflow or bypass discharge of wastewater to the surface waters of the State. 7. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the requirements of this permit and the approved plans and specifications. Mail the certification to the Permits and Engineering Unit, P.O. Box 27687, Raleigh, N.C. 27611-7687. 8. A copy of the approved plans and specifications shall be maintained on file by the permittee for a minimum of five years from the date of issuance. 9. Adequate separation distances of not less than 12 inches (vertical) shall be provided between the proposed sewer pipe and storm sewers, and 18 inches (vertical) or 10 feet (horizontal) from water mains. A minimum of 18 inches vertical separation shall also be maintained in any benched trenches. Ferrous sewer pipe shall be used where these minimum separations cannot be maintained. 10. The pipe shall maintain a slope such that the velocity of flow shall be greater than 2.0 feet per second and less than 10 feet per second. 11. The sewer lines not approved by this permit, that were included on the plans, are noted on the plans. 12. Please resubmit these approved plans with applications for permits or amendments for future development of this property. These will be returned to you.
