Industrial Wastewater Treatment and Disposal Facility Beaufort County Bear Mr. Singh: In accordance with your letter dated November 23, 1988, we are forwarding herewith Permit No. WQ0000628, dated January 17, 1989, to Flanders Filters, Inc. for the construction and operation of the subject wastewater treatment and spray irrigation facilities. This permit shall be effective from the date of issuance until July 1, 1992, shall void Permit No. 4276-R2, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed with the Cffice of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. Beaufort County FOR THE continued operation. of 30,000 GPD Wastewater Treatment Facility consisting of 18,000 gallon equalization/dose tank, six (6) filter beds with total area of 225 square feet, dual 40 GPM dosing pumps and various piping, a 9,000 gallon filtrate storage tank, a 45,000 gallon storage basin, dual 20 GPM irrigation pumps with a 8.24 acre spray irrigation field to serve Flanders Filters, Inc. with no discharge to the surface waters of this State. This permit shall be effective from the date of issuance until July 1, 1992, shall void Permit No. 4276-R2, and shall be subject to the following specified conditions and limitations: 1. This permit shall become voidable unless this facilities are constructed in accordance with the approved plans, specifications, and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities shall be properly maintained and operated at all times. 4. This permit is not transferable. 5. This permit shall become voidable in the event of failure of the soil to adequately absorb the wastes and may be rescinded unless the facilities are installed, maintained, and operated in a manner which will protect the assigned water quality standards of the surface waters and groixnd waters. 6. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional. or replacement wastewater treatment or disposal facilities. 7. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited, 8. The treatment facility shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters which will render them unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions or failure of the irrigation area to adequately absorb the wastewater, the Permittee shall take such immediate corrective action as may be required by the Division of Environmental Management. 9. A suitable year round vegetative cover shall be maintained. 10. Irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff. 11. Adequate measures shall be taken to prevent wastewater runoff from the spray field. 12. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate, 13. The Permittee shall employ a certified wastewater treatment plant operator -to be in responsible charge of the wastewater treatment facilities. Such operator must hold a certificate of the grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. 14. The application rates shall not exceed .67 inches per acre per week. 15. The sludge generated from these treatment facilities must be disposed of in accordance with General Statute 143-215,1 and in a manner approvable by the North Carolina Division of Environmental Management. 16. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 17. No type of wastewater other than that from Flanders Filters, Inc. shall be sprayed onto the irrigation area. 18. Adequate provisions shall be taken to prevent wind erosion and surface runoff from conveying pollutants from the sludge application area onto the adjacent property or into the surface waters. 19. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 20. The spray system shall be inspected monthly to make certain that the nozzles are operating properly , that the vegetative growth allows for proper spray patterns, that the soil is absorbing the disposed treated wastewater with no surface runoff, and that no objectionable odors are being generated. 21. Flanders Filters, Inc. shall have submitted a report on the sodium absorption j ratio (SAR) of their wastewater and its effects on the spray field by January 1, 1988 to the Division of Environmental Management, Washington Regional Office. 22. All the cutting from the cover crop shall be removed from site. 23. Total Nitrogen in the effluent shall be sampled for on a monthly basis and reported to the Washington Regional Office. 24, The Washington Regional Office shall be notified 24 hours prior to any construction so that the inspection program can be carried out. 25. Within 90 days of permit issuance, two (2) monitor wells, one (1) upgradient and one (1) downgradient, shall be installed to monitor groundwater quality. The location and construction details for these wells shall be by approval of the Washington Regional Office, from which a well construction permit must be obtained. 26. The two (2) new monitor well shall be sampled initially after construction and thereafter every March, July and November for the following parameters: NO pH ToLl Amonia TOC TDS phenol Water Levels SO4 Zinc, Aluminum The measurement of water level must be made prior to sampling for the remaining parameters. The results of the sampling and analysis shall be sent to the N.C. Division of Environmental Management on Form GW -59 (Compliance Monitoring Report Form) every April, August and December. 27. All existing monitor wells (MW -k, DIW-8, and MW -9) shall be sampled every March, July and November for the parameters described in Condition No. 26 above. The results of the sampling and analysis shall be sent to the N.C. Division of Environmental Management on Form GW -59 [Compliance Monitoring Report Form]'every April, August, and December. 28. The measuring points of all existing and new monitoring wells shall be surveyed relative to Mean Sea Level (MSL). Specifically, if the measuring point for water level determination is the top of all the PVC well casing, the top of the casing shall be surveyed relative to MSL. The depth to water in each well shall be measured from the surveyed point on the top of the PVC casing. The water level elevation shall then be determined relative to MSL. 29. Any additional groundwater quality monitoring, as deemed necessary by the Department, shall be provided. Permit issued this the 17th day of January, 1989.
