JAY GRAINGER, WWTP SUPERINTENDENT CITY OF SANFORD POST OFFICE BOX 338 SANFORD, NORTH CAROLINA 27330 Subject: Attachment B Certification Permit No. WQ0000543 City of Sanford City of Sanford Residuals Land Application Program Land Application of Residual Solids (503) Lee County Dear Mr. Grainger: In accordance with your permit modification application package received on October 4, 2005 for review under the Division of Water Quality's (Division) express review program as well as the additional information received on October 6, 2005; October 20, 2005; and October 21, 2005; we are forwarding herewith a modified Attachment B, certified November 4, 2005, to City of Sanford for the subject residuals land application program. This certification is being issued to modify Attachment B to approve 332.7 net acres of new land application sites, consisting of three sites (i.e., Site Nos. NC -CH -17, NC -CH -101, and NC -MR -02) over two counties (i.e., Chatham and Moore), for use under the residuals land application program. Site Nos. NC -CH -17 and NC -MR -02, which have four fields and three fields respectively, are being approved for the first time with this certification. Site No. NC -CH -101, which has 15 fields, were previously approved for use under the Town of Cary's residuals land application program (i.e., Permit No. WQ0000691). With the additions, this program has a total of 973.9 net acres of land available for residuals application. Please replace the existing Attachment B of Permit No. WQ0000543 that was previously certified on March 14, 2005 and subsequently corrected on March 28, 2005, with this modified Attachment B. Note that only the vicinity and buffer maps for any new and modified land application sites are attached to this modified Attachment B. It is the Permittee's responsibility to ensure that a complete permit is created by inserting this modified Attachment B and vicinity and buffer maps into Permit No. WQ0000543, issued on March 14, 2005, and making sure to retain any vicinity and buffer maps for any existing and unmodified land application sites. As always, remember to take the time to review this modified Attachment B thoroughly, as some of the conditions contained therein may have been changed since the last certification. Note that this modified Attachment B is hereby incorporated by reference into Permit No. WQ0000543, issued on March 14, 2005. If any parts, requirements, arid/or limitations contained in this certification or modified Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 calendar days following receipt of the certification. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such demands are made, this permit shall be final and binding.
