This permit shall be effective from the date of issuance until January 31, 1994, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150E of North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this permit shall be final and binding. One (1) set of approved plans and specifications is being forwarded to you. Box 27687, Raleigh, North Carolina 27611-7687 Telephone 919.733-7015 An Equal Opportunity Affirmative Action Employer NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT RALEIGH P E R M I T For the discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO Lincoln County Schools Lincoln County FOR THE construction and operation of a 7,500 GPD subsurface wastewater treatment and disposal facility consisting of a 9,000 gallon septic tank and 1,500 gallon, dosing tank in one unit, dual 75 GPM submersible pumps, approximately 490 linear feet of 3 -inch force main, two distribution boxes, two subsurface disposal fields with approximately 3,125 linear feet of 3 -foot nitrification trench each, two monitoring wells and all associated piping and appurtenances to serve Union Elementary School, pursuant to the application received October 3, 1988, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this permit. This permit shall be effective from the date of issuance until January 31, 1994, and shall be subject to the following specified conditions and limitations! 1. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifyi>>g that the permitted facility has been installed in accordance with the approved plans and specifications. Mail the certification to the Permits and Engineering Unit, PO Box 27687, Raleigh, NC 27611-7687. 2. This permit shall. become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. 3. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 4. The facilities shall be properly maintained and operated at all times. 5. This permit is not transferable. 6. Upon the availability of a municipal or .regional sewerage collection system, the subject wastewater treatment facility shall he abandoned and all wastewater discharged into the municipal or regional sewerage system. 7. This permit shall become voidable in the event of failure of the soil to adequately absorb the wastes and may be rescinded unless the facilities are installed, maintained, and operated in a manner which will protect the assigned water quality standards of the surface waters and ground waters. 8. In the event 'that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or disposal facilities. 9. The Mooresville Regional Office, phone no. 704/663-1699 shall be notified at least twenty-four (24) hours in advance of backfilling of the installed system so that an in-place inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. 10. The sludge generated from these treatment facilities must be disposed of in accordance with General Statute 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. 11. A suitable year round vegetative cover shall. be maintained. 12. Diversion or bypassing of the untreated wastewater from the treatment facility is prohibited. 13. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the Permit for such period of time and under such conditions and limitations as it may deem appropriate. 14. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 15. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 16. No type of wastewater other than that from Union Elementary School shall be made tributary to the subsurface disposal system. 17. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal maintenance is being performed. 18. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 19. An acceptable reserve area, equal in size to the approved disposal area, shall be maintained at all times. 20. A set of approved plans and specifications for the subject project must be retained by the applicant for the life of the project. 21. The application rate shall not exceed 0.4 gallons/sq. ft./day. 22. Prior to beginning waste disposal operations, two (2) monitor wells, one (1) upgradient and one (1) downgradient, shall be installed to monitor groundwater quality. The location and construction details for these wells shall be approved by the Mooresville Regional Office, from which a well construction permit must be obtained. 23. The monitor wells shall be sampled initially after construction (and prior to waste disposal operations) and thereafter every March, July, and November for the following parameters: NO3 TOC Chloride Total Ammonia Total Coliforms Water Level pH TDS The measurement of seater level must be made prior to sampling for the remaining parameters. The results of the sampling and analysis shall be sent to the N.C, Division of Environmental Management on dorm GW -59 (Compliance Monitoring Report Form) every April, August and December. 24. Any additional groundwater quality monitoring as deemed necessary by the Department, shall be provided. Permit issued this the 31st day of January, 1989.
