Box 905 Carthage, NC 28327 SUBJECT: Permit No. WQ0000511 Moore County Solid Waste Department Solid Waste Site No. 2 West End, Moore County Dear Mr. Kidd; In accordance with the permit application filed by the Moore County Solid Waste Department on September 29, 1988, and supporting information supplied by the consulting engineer, Mr. Charley Shuffler, we are forwarding herewith Permit No. WQ0000511 to the Moore County Solid Waste Department. This permit is for the construction and operation of a septic tank -nitrification field system to serve the maintenance attendant station at the Moore County Solid Waste Site No. 2 near West End. This permit shall be effective from the date of issuance until rescinded and shall be subject to the conditions and limitations therein. If any parts, requirements, or limitations contained in this perillit are unacceptable to you, you have the right to request an adjudicatory hearing sy upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150E of North Carolina General Statutes, and filed with the office of Administra- tive Hearing, P.Q. Drawer 11666, Raleigh, NC 27604. Unless such demands are made, this permit shall be final and binding. Ramsey '0t NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT RALEIGH P E R M I T For the Discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provision of -Article 21 of Chapter 143, General Statutes of North Carolina as amended and other applicable Laws, Rules, and Regulations, PERMISSION IS HEREBY GRANTED TO Moore County Solid Waste Department Solid Waste Site No. 2 West End, Moore County FOR THE construction and operation of a septic tangy: -nitrification field system consisting of a 1000 -gallon precast septic tank, a distribution box, and a total of 100 L.F. of nitrification line installed in 3 --foot trenchs, pursuant to the application received October 3, 1988, and in conformity with the project plans, specifications, and other supporting data, subsequently filed and approved by the Department of Natural Resources and Community Development and considered a part of this Permit. This Permit shall be effective from the date of issuance until rescinded and shall be subject to the following specified conditions and limitations; 1. This Permit shall become voidable unless the facility is constructed in accordance with the approved plans, specifica- tions, and other supporting data. 2. This Permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facility shall be properly maintained and operated at all times. 4. This Permit is not transferable. 5. Solids, sludges, or other pollutants removed or resulting from the wastewater storage facility shall be contained and disposed of in such a manner as to prevent any contamination of the surface or groundwater of the State. Moore County Solid Waste Department Solid Waste Site No, 2 Page 2 6. An acceptable reserve area, equal in size to the approved disposal area, shall be maintained at all times. 7. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facility. 8. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal maintenance is being performed. 9. The facility shall be effectively maintained and operated as a nondischarge system to prevent the discharge of any wastewater resulting from the operation of this facility. 10, The issuance of this Permit shall not relieve the Permittee of the responsibility for damages to surface or gr'oundwaters resulting from the operation of this facility. 11. Diversion or bypassing of the untreated wastewater from the treatment facility is prohibited. 12. Monitoring wells shall be constructed and sampling 'schedules arranged as required by the Division of Environmental Management and in accordance with 15 NCAC 2C .0100. 13. Any monitoring deemed necessary by the Division of Environmental Management to insure surface and groundwater protection will be established and an acceptable sampling reporting schedule shall be followed. 14. The Fayetteville regional Office, phone number (919) 486-1541, shall be notified at least twenty-four (24) hours in advance of backfilling of the installed subsurface disposal system so that an in-place inspection can be made of said system prior to backfilling. Such notification to the Regional Supervisor shall be made during the normal office hours from 8 a.m. until 5 p,m. Monday through Friday, excluding state holidays. 15. In event the facility fails to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or a disposal facility. 16. This Permit shall become voidable in the event of failure of the soil to adequately absorb the wastes and may be rescinded Moore County Solid Waste Department Solid Waste Site No. 2 Page 3 unless the facility is installed, maintained, and operated its a manner which will protect the assigned water quality standards of the surface waters and groundwaters. 17. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the approved plans and specifications. 18. All surface drainage on this site shall be diverted away from any portion of the nitrification field or designated repair area. 19, The nitrification field shall be properly graded and seeded to ensure prompt runoff of surface water. 20. The invert of the nitrification lines shall not exceed a depth of 36 inches below natural ground level. Permit issued this 6th day of October, 1988. INSPECTION OF PROPOSED SEPTIC TANK -NITRIFICATION FIELD SITE 1. Place Visited: Proposed Solid Waste Disposal Site No. 2 near West End - Seven Lakes, Moore County. 2. Date of Investigation: September 1, 1988 3. Report Prepared By: I.en Averitte, Environmental Technician, Fayetteville Regional Office 4. Persons Contacted and Telephone Number: Mr. Farrell Kidd, Manager, Moore County Solid Waste Department and Mr. Sam Fields, Moore County Health Department 5. Directions to Site: The proposed disposal site is located on the east side of SR 1238, approximately 112 mile from NC 211, between West Enol and Seven Lakes. 6. Size: The total site area is approximately 4 112 acres. 7. Topography: Slight to moderate slopes. 8. Location of Nearest Dwelling: Approximately 300 ft. 9. Distance to Nearest Well: The nearest well will be a supply well onsite, approximately 150 ft. from the proposed system. 10. Distance to Nearest Stream: Approximately 1000 ft. PART II -- DESCRIPTION OF TREATMENT WORDS 1. Type of wastewater: Domestic wastewater from a single rest room provided for an attendant at the disposal site only. Type of Treatment: Wastewater is to be treated in a 1000 -gallon precast septic tank, as approved by the Sanitary Engineering Branch, Division of Health Services, DHR. The nitrification field is to contain 300 square feet of absorption area, with 4 --inch distribution tile. Flow is to be evenly divided through a distribution box. Soils at the disposal site have been evaluated and found to be similar to Lakeland sands, with a hydraulic loading capacity of -crp to 1 gal/ft /day. No evidence of a seasonal water table was found within a four -foot auger hole. Actual water use at the site will likely be much less than 120 gpd. At a rate of 150 gpd, this would provide a loading of .5 g/ft /day, which should present no problems with the system as designed. Satisfactory repair area is available should it ever become necessary. PART III - OTHER PERTINENT INFORMATION 1. Water Supply: Water is to be provided by a well onsite. 2. Soil conditions have been evaluated by the Moore County Health Department and found satisfactory for the intended use. 3. The Groundwater section has indicated no objection to this construction. PART IV - PLANS AND SPECIFICATIONS REVIEW Plans and specifications for this project have been carried out in accordance with existing Division of Environmental Management requirements. FART V - EVALUATION AND RECOMMENDATION It is the recommendation of this office that a permit be issued for the construction and operation of the previously described nondischarge facility. This recommendation is based on the fact that the site appears suitable for the size and type of system proposed.
