Dear Mr. Lewis:
In accordance with your application received Septomber 21, 1988, we are
forwardirr.g herewith Permit No. WQ0000508, dated Decembe:,- 13, 1988, to Waste
Management of North America, Inc. for the construction andoperation of the subject
wastewater facilities.
This permit shall be effective from --the date of issuance until December 31,
1993, and shall be subject to the conditions and limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This'regL.est must
be in the form of a written petition, conforming to Chapter 150B of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this
permit shall be final and binding,

For the discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
construction and operation of a 400 GPD septic tank system consisting of a truck
wash pad with canopy, a 1,500 gallon septic tank which will be used as a oil/water
separator and five (5) drain lines each 80 linear feet to serve Waste Management's
ruck Stop located at Union and Beaty Road with no discharge to the surface waters of
the state, pursuant to the application received September 21, 1988, and in
conformity with the project plan, specifications, and other supporting data
subsequently filed and approved by the Department of Natural Resources and Community
Development and considered a part of this permit.
This permit shall be effective from the date of issuance until December 31, 1993,
and shall be subject to the following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable.
5. Construction of the sewers shall be so scheduled so as not to interrupt service
by the existing utilities nor result in an overflow or bypass discharge of
wastewater to the surface waters of the State.
�. Upon completion of construction and prior to operation, of this permitted
I facility, a certification must be received from a professional engineer
certifying that the permitted facility has been installed in accordance with the,
requirements of this permit and the approved plans and specifications. Mail the
certification to the Permits and Engineering Unit, P.D. Box 27687, Raleigh, N.C.
27611-7687.
7. A set of approved plans and specifications for the subject project must be
retained by the applicant for the life of the project.
8. Thirty (30) days following start --up of the system, an effluent sample shall be
obtained from a point in the system (ie. the splitter box) after the septic tank
and prior to disposal in the nitrification lines, and analyzed for the following
parameters:
Oil & Grease (Method 503 A; Standard Methods, 15th ed.)
Purgeable Halocarbons (EPA Method 601)
MBAS
Orthophosphate
Additional effluent samples shall be obtained in March, July and November for a
period of one (1) year, and analyzed for those parameters specified above.
The results of the sampling and analysis shall be sent to the Mooresville
Regional Office, Groundwater Section, every April, August and December.
No crankcase oils or degreasing solvent waste s'.ialdl be discharged to the
wastewater disposal system.
10. This permit shall become voidable in the event of failure of the soil to
adequately absorb the wastes aid may be rescinded unless the facilities are
installed, maintained, and operated in a manner which will protect the assigned
water quality standards of the surface waters and ground waters.
11. In the event that the facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the permittee shall take such immediate
corrective action as may be required by this Division, including the construction
of additional or replacement wastewater treatment or disposal facilities.
12. The Mooresville Regional Office, phone no. 704/663-1699 shall be notified at
least twenty-four (24) hours in advance of operation of the installed
facilities so that an in-place inspection can be made. Such notification to the
regional supervisor shall be made during the normal office hours from 8:00 a.m.
until 5:00 p.m. on Monday through Friday, excluding State Holidays.
13. The sludge generated from these treatment facilities must be disposed of in
accordance with General Statute 143-215.1 and in a manner approvable by the North
Carolina Division of Environmental Management.
+. The facilities shall be effectively maintained and operated at all times so that
there is no discharge to the surface waters, nor any contamination of ground
waters which will render them unsatisfactory for normal use. In the event that
the facilities fail to perform satisfactorily, including the creation of nuisance
conditions or failure of the irrigation area to adequately absorb the wastewater,
the Permittee shall take such immediate corrective action as may be required by
the Division of Environmental Management.
15. The application rates shall not exceed 0.333 GPD/square feet.
16. The issuance of this permit shall not relieve the Permittee of the responsibility
for damages to surface or groundwaters resulting from the operation of this
facility.
17. No traffic or equipment shall be allowed on the disposal area except while
installation, occurs or while normal maintenance is being performed.
18. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee
to insure proper operation of the subject facilities.
19. The Permittee is responsible for the operation and maintenance of the entire
treatment and disposal system including, but not limited to, the following item!
a. The septic tank shall be checked every six (6) months and pumped out as
required.
0. Failure of the Permittee to properly operate this system is subject to a penalty
up to $10,000 per day.
21. Any additional groundwater quality --monitoring as deemed necessary by the
Department, shall be provided.
