Helms Director Telephone 919 733-7015 SUBJECT: Permit No. 6.494--F— L'io Q 000 cl 9 0 Oak Island Associates Oak Island Villas Condominiums Wastewater Treatment Facilities Brunswick County Dear Mr. Atkinson: In accordance with your letter of request received February 15, 1984, we are forwarding herewith Permit No. 6906-R, dated March 13,1984, to Oak Island Associates for the operation of the subject existing nondischarge type waste- water treatment and disposal facilities. The original Permit No. 6906, issued October 14, 1981, and the Amendment to Permit No. 6906, issued September 27, 1983, are hereby rescinded. This permit shall be effective from the date of issuance until March 31, 1989, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing before a hearing officer upon written demand to the Director within thirty (30) days following receipt of this permit, identifying the specific issues to be con- tended. Unless such demand is made, this permit shall be final and binding. One (1) set of approved plans and specifications is being forwarded to you. Box 27687 Raleigh, N.C. 27611-7687 An EqualOppartunityAffirmative Action Employer NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF NATURAL RESOURCES AND COMMUNITY DEVELOPMENT RALEIGH P E R M I T For the Discharge of Sewage, Industrial Wastes, or Other Wastes In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO Oak Island Associates Brunswick County FOR THE operation of a 50,000 GPD nondischarge type wastewater treatment facility consisting of a bar screen/flow splitter/flow measuring device, dual 25,000 gallon capacity aeration tanks, dual clarifiers, dual aerated sludge holding tanks, tertiary filter with dual beds, a chlorine contact tank with tablet chlorinator, a dose tank with dual pumps, dual rotary distributors with approximately 2,830 square feet of bed each, a standby power generator, approximately 645 lineal feet of 8 -inch gravity sewer, 675 lineal feet of 4 -inch force main, and a 87 GPM pump station with dual pumps, standby power, and high-water alarm to serve Oak Island Villas Condominiums with no discharge of wastewater to the surface waters of the State, pursuant to the letter of request received February 15, 1984, and in conformity with the project plans, specifications, and other supporting data, subsequently filed and approved by the Department of Natural Resources and Community Development and con- sidered a part of this permit. This permit shall be effective from the date of issuance until March 31, 1989, and shall be subject to the following specified conditions and Limitations: 1. This permit shall become voidable unless the facilities are constructed in accordance with the approved plans, specifications and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities shall be properly maintained and operated at all times. 4. This permit is not transferable. Permit No. 6906-R Page Two 5. This permit shall become voidable in the event of failure of the soil to adequately absorb the wastes, and may be rescinded unless the facilities are installed, maintained and operated in a manner which will protect the assigned water quality standards of the surface waters, and prevent any contamination of the ground waters which will render them unsatisfactory for normal use. 6. In event the facilities fail to perform satisfactorily, the Permittee shall take such immediate corrective action as may be required by this Department. 7. Solids, sludges, or other pollutants removed or resulting from the waste- water treatment process shall be contained and disposed of in such a manner as to prevent any contamination of the surface or ground waters of the State. S. This is a Class II Wastewater Treatment Plant and the person in responsible charge must hold a valid Grade II Certification. 9. Diversion or bypassing of the untreated wastewater from the treatment faciliites is prohibited. 10. This permit shall become voidable unless the agreement between Oak Island Associates, Oak Island Beach Condominiums Homeowners Association, and the North Carolina Division of Environmental Management for the collection and final treatment of domestic wastewater is in full force and effect. 11. The Permittee shall provide and maintain monitoring facilities and a ground water monitoring program such as may be required by the Division of Environmental Management. 12. The portion of the project site designated on the approved plans for "waste treatment and green area", and consisting of 3.1 acres, must be maintained either in its natural state or modified only by planting vegetative cover of grasses or low growing shrubbery, else this Permit becomes voidable. No structures or paved surfaces other than those associated with the approved wastewater treatment and disposal facilities shall be constructed within the "waste treatment and green area." 13. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein and, if indicated, will extend the Permit for such period of time and under such conditions and limita- tions as it may deem appropriate. 14. The Permittee shall submit periodic self --monitoring reports to the Division of Environmental Management based on the attached sampling schedule to insure surface and groundwater protection.
