Oak Island Beach Condominiums Wastewater Treatment and Collection Facilities Brunswick County Dear Mr. Clifton: In accordance with your application received September 30, 1981, we are forwarding herewith Permit No. 6906, dated October 14, 1981, to Oak Island Beach Condominiums for the construction and operation of the subject non - discharge type waste treatment facilities. This permit shall be effective from the date of issuance until rescinded, and shall be subject to the conditions and limitations as specified therein. If any parts, requirements, or .limitations contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing before a hearing office upon wirtten demand to the Director within thirty (30) days following receipt of this permit, identifying the specific issues to be con- tended. Unless such demand is made, this permit shall be final and binding. One (1) set of approved plans and specifications is being forwarded to you. This permit shall become voidable in the event of failure of the soil to adequately absorb the wastes, and may be rescinded unless the facilities are installed, maintained and operated in a manner which will protect the assigned water quality standards of the surface waters, and prevent any contamination of the ground waters which will render them unsatisfactory for normal use. 6. In event the facilities fail to perform satisfactorily, the Permittee shall take such immediate corrective action as may be required by this Depart- ment. 7. Solids, sludges, or other pollutants removed or resulting from the waste- water treatment process shall be contained and disposed of in such a manner as to prevent any contamination of the surface or ground waters of the State. 8. This is a Class II Wastewater Treatment Plant and the person in responsible charge must hold a valid Grade 11 Certificate. 9. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. 10. This permit shall become voidable unless the agreement between Oak Island Beach Condominiums, Inc., Oak Island Beach Condominium Homeowners Associa- tion, and the North Carolina Division of Environmental Management for the collection and final treatment of domestic wastewater is in full force and effect. 11. The Permittee shall provide and maintain monitoring facilities and a ground water monitoring program such as may be required by the Division of Environmental Management. 12. The portion of the project site designated on the approved plans as "waste treatment site" and consisting of 2.87 acres, must be maintained either in its natural state or modified only by planting vegetative cover of grasses or low growing shrubbery, else this Permit becomes voidable. No structures or paved surfaces other than those associated with the approved wastewater treatment and disposal facilities shall be constructed within the "waste treatment site". The attached monitoring schedule has been developed by this Office and should provide sufficient monitoring data to determine any impacts on surface water and groundwater quality which are due to activities at your facility. If any parts or requirements contained in this monitoring schedule are unacceptable to you, please notify this office in writing within 30 days.
