Reference is made to your request for rescission of the subject Surface Irrigation Permit. Staff from the
Washington Regional Office has confirmed that this Permit is no longer required. Therefore, in accordance
with your request, Surface Irrigation Permit No. WQ0000489 is rescinded, effective immediately.
Within 60 days, the three existing monitoring wells (MW -4, MW -5, and MW -6) shall be abandoned in
accordance with 15A NCAC 2C .0113. A well abandonment record (GW -30) for each well should be
submitted to the Division and the Washington Regional Office within 30 days of well abandonment.
If in the future, you wish again to operate a non -discharge system, you must first apply for and receive a new
Surface Irrigation Permit. Operating a non -discharge system without a valid Surface Irrigation Permit will
subject the responsible party to a civil penalty of up to $ 10,000 per day.
