Winston-Salem This letter is in reference to the Notice of Violation and Revocation letter dated
919/896-7007 December 23, 1991 which you received on December 28, 1991. You were informed in
the previous letter that your permit would be revoked in 60 days if the annual
administering and compliance monitoring fees for 1990 & 1991 were not received
during that period_ The 60 day period has passed and we have not received your
payment. Therefore, your permit was revoked effective February 28, 1992.
Please be advised that engaging in the land application of sludge activities
without a valid permit will subject the owner to a civil penalty of up to $10,000 per
day. If you wish to operate this facility in the future, you must first apply for and
receive a new permit. Failure to obtain a valid Non discharge Permit for such activities
will subject the owner to a civil penalty of up to $10,000 per day.

Dear Mr. Black:
In accordance with your application received June 16, 1.988, we are forwarding
herewith Permit No. WQ0000482, dated September 28, 1988, to Coastal, Soil
Enrichment, Inc. for the construct] -on and operation of the subject wastewater sludge
land application program.
This permit shall be effective from the date of issuance until September 30,
1993, and shall be subject to the conditions and limitations as specified therein.
If any parts, requirements, or limitations contained .in this permit are
unacceptable to you, you have the right to .request an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request must
be in the form of a written petition, conforming to Chapter. 150B of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer 1.1666, Raleigh, North Carolina 27604. Unless such riemands are-, made this
permit shall be final and binding.

In accordance with the provisions of Article 21 of Chapter 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rulp.s, and Regulations

and shall be subject to the following specified conditions and limitations:
1. This permit is effective only with respect to the; natnrs, a.nd volume of wastes

2. The facilities shall be properly maintained and operated at: 91.1 times.
3. This permit is not transferable.

6. This permit does not authorize the land application of slrndge. This permit must
be amended to include each sludge source.
7. The sludge analyzes for each sludge source shall include TOX in addition to PAN,
P, K, all applicable metals and E.P. TOX Leachate.
8. Any monitoring deemed necessary by the Division of Environmental Management to
insure surface and ground water protection will be established and an acceptable
sampling reporting schedule shall be followed.
9. This permit shall become voidable in the event of failure of the soil to
adequately absorb the wastes and may be rescinded unless the facilities are
installed, maintained, and operated in a manner which will. protect the assigned
water quality standards of the surface waters and ground waters.
10. In the event that the facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take such immediate
corrective action as may be required by this Division, including the construction
of additional or replacement wastewater treatment or disposal facilities.
11. A suitable year round vegetative cover shall be maintained.
12. The Permittee, at least six (6) months prior to the expiration of this permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the facilities described therein, and [i= warranted, will
extend the Permit for such period of time and under such conditions and
limitations as it may deem appropriate.
13. The issuance of this permit shall not relieve the Permittee of the responsibility
for damages to surface or groundwaters resulting from the operation of this
facility.
14. Sludge shall not be applied in inclement weather or until 24 hours following a
rainfall event of 1/2 -inch or greater in 24 hours. Emergency sludge disposal
measures shall be approved by the Division of Environments] Management.
15. Sludge shall be uniformily applied to avoid runoff and ponding.
16. A recorded log of sludge applications including amounts applied, dates, method of
application, and site location shall be maintained at by the Permittee as a
permanent record.
17. The site shall be adequately limed to a soil pH of 6.5 prior to sludge
application.
18. No crops for direct human consumption shall be raised rni the.so sites for a period
of 18 months following sludge application.
19. No liquid sludge shall be surface applied within a 100 -.foot: bliffer_ zone of any
water course or surface drainage area.
20. No liquid sludge shall be applied by subsurface injection within a 25 -foot buffer
zone of any water course or surface drainage area.
21. A 50 -foot buffer shall be maintained between drain.ageways, a 100 -foot buffer
shall be maintained between each landowner's property lines, and a 400 -foot
buffer shall be maintained between any residence and any applied sludge.
22. Animals should not be grazed on sludge applied land within a 30 -day period
following the sludge application. Application sites that are to be used for
grazing shall have fencing that will be used to prevent access during the 30 -day
periods after each application.
23. No root crops shall be grown on these sites within three (3) years of the last
application of Sludge.
24. Signs shall be posted at each site notifying the public that: sludge has been
land applied, unless all sludge is soil incorporated within 48 hours of
application.
25. Sludge application shall not exceed:

26. A copy of the approved plans and specifications shall be maintained on file by
the permittee for a mini -mum of five years from the date of issuance.
