In accordance with your permit modification application package received on April 1, 2003 as well as
the additional information received on June 11, 2003 and June 24, 2003; we are forwarding herewith a
modified Attachment A, certified June 30, 2003, to the Lewis Farms of Burclaw, Inc. for the subject residuals
]and application program.
This certification is being issued to modify Attachment A to approve residuals from two additional
residuals source -generating facilities (i.e,, package -plant type) for land application under this program. These
facilities, which are located in Onslow County and will contribute a maximum of 16.00 dry tons per year to
the volume of residuals allowed to be land applied, are as follows:

Also, at your request, Attachment A is being modified with this certification to delete two residuals
source -generating facilities (i.e., package -plant type) previously approved for land application under this
program. These facilities, which are located in New Hanover County and were approved for a maximum of
20.64 dry tons per year to the volume of residuals allowed to be land applied, are as follows:

Please replace the existing Attachment A of Permit No. WQ0000455 that was previously,
certified on February 7, 2003, with this modified Attachment A. Note that only the vicinity maps for any
new and modified residuals source -generating facilities are attached to this modified Attachment A. It is the
Permittee's responsibility to ensure that a complete permit is created by inserting this modified Attachment
A and vicinity maps into Permit No. WQ0000455, issued on February 7, 2003, and making sure to retain any
vicinity maps for any existing and unmodified residuals source -generating facilities.
This modified Attachment A shall be effective from the date of certification; shall void Attachment A
of Permit No. WQ0000455 that was previously certified on February 7, 2003, and shall be subject to the
conditions and limitations as specified therein and well as those specified in the above -referenced permit. As
always, remember to take the time to review this modified Attachment A thoroughly, as some of the
conditions contained therein may have been changed since the last certification. Note that this modified
Attachment A is hereby incorporated by reference into Permit No. WQ0000455, issued on February 7, 2003.
If any parts, requirements, and/or limitations contained in this certification or modified Attachment A
are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days
following receipt of the certification. This request shall be in the from of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at
6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such demands are made, this permit
shall be final and binding.
