In accordance with your permit major modification request received December 5, 2012, we are
forwarding herewith the modified Attachment A, certified December 21, 2012, to Lewis Farms and
Liquid Waste, Inc. for the subject residuals management program.
This certification is being issued to modify Attachment A by adding a new residuals source
(Town of Clarkton — NCO21610) for 32 dry tons per year. Please replace. the existing Attachment A of
Permit No. WQ0000455 that was previously certified on February 2, 2012, with this modified
Attachment A.
This modified Attachment A shall be effective from the date of certification; shall void
Attachment A of Permit No. WQ0000455 that was previously certified on February 2, 2012; and shall be
subject to the conditions and limitations as specified therein and well as those specified in the above -
referenced permit. Note .that this modified Attachment A is hereby incorporated by reference into Permit
No. WQ0000455, issued on February 2, 2012.
If any parts, requirements, and/or limitations contained in this certification or modified
Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the form of a written
petition, conforming to Chapter 150E of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.
