Dear Ms. Zinn:
In accordance with your application received February 22, 1989, we are
forwarding herewith Permit No. WQ0000365, dated March 6, 1989, to Cazwell
Development, Inc. for the continued operation of the subject pump and haul
wastewater disposal system.
This permit shall be effective from the date of issuance until June 15, 1989,
and shall be subject to the conditions and limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request must
be in the form of a written petition, conforming.to Chapter 150B of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this
permit shall be final and binding.

For the discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter. 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO
Cazwell Development, Inc.
Durham County
FOR THE
continued operation of a 1,080 GPD pump and haul disposal system from an on-site
collection manhole at Montvale Subdivision and the discharge of collected domestic
wastewater to the Farrington Road Wastewater 'Treatment Plant, pzarsuant to the
application received September 14, 1988, and in conformity with the project plan,
specifications, and other supporting data subsequently filed and approved by the
Department of Natural. Resources and Community Development and considered a part of
this permit.
This permit shall be effective from the date of issuance until ,lune 15, 1989, and
shall be subject to the following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are operated in
accordance with the approved pians, specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
3. The facilities shall he properly maintained and operated at all times.
4. The sewage and wastewater collected by this system shall be treated in the
Farrington Wastewater Treatment Plant prior to being discharged into the
receiving stream.
5. This permit is not transferable.
6. This permit shall become voidable unless the agreement between Cazwell
Development, Inc. and the City of Durham for the collection and final treatment
of wastewater is in full force and effect.
7. The Permittee is liable for any damages caused by a spill or failure of the pump
and haul operations.
8. The issuance of this permit shall. not relieve the Permittee of the responsibility
for damages to surface or groundwaters resulting from the operation of this
facility.
9. No wastewater other than that from Montvale Subdivision shall be included in this
pump and haul. operation. No other homes shall be included in this permit other
than those mentioned in the original application dated September. 14, 1988.
10. The facilities shall be effectively maintained and operated as a non -discharge
system to prevent the discharge of any wastewater resulting from the operation of
this facility.
11. This permit shall expire on June 15, 1989 and shall not be renewed.
