In accordance with your application received September 14, 1988, we are
forwarding herewith Permit No. WQ0000365, dated September 27, 1.988, to Caswell
Development, Inc. for the operation of the subject pump and hanl wastewater disposal
system.
This permit shall be effective from the elate of issuance until March 30, 1989,
and shall be subject to the conditions and limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt- of tbis permit. This request must
be in the form of a written petition, conforming to Chapter 15013 of North Carolina
General Statutes, and filed with the Office of Adm-.misttrat.ive Hearings, Post Office
Drawer 11666, Raleigh, North Carolina 27604. Unless stich demands are made this
permit shall be final and binding.

and shall be subject to the following specified conditious and limitations:
I. This permit shall become voidable unless the facilities are operated in
accordance with the approved plans, specifications and nl:h.er supporting data.
2. This permit is effective only with respect to the. rrntIM-0.. .and volume of wastes
described in the application and other supporting dntn.
3. The facilities shall be properly maintained and opor;rod nt nil times.
4. The sewage and wastewater collected by this 5ysi:t-.m slum l be treated in the
Farrington. Wastewater Treatment Plant prior to ho ing discharged into the
receiving stream.
5. This permit is not transferable.
6. This permit shall become voidable unless the agrn.c,merrt between Caswell
Development, Inc. and the City of Durham for the re l l oci-ion arrcl final. treatment
of wastewater is in full force and effect.
