Dear Mr. Mitsch:
In accordance with your application received September 2, 1988, we are
forwarding herewith Permit No. WQ00003.57, dated Decembe.:- 8, 1988, to Liselotte
Rest Home for the construction and operation of the subject wastewater collection,,
treatment, and disposal. facilities.
This permit shall be effective from the date of issuance until November 30,
1993, and shall be subject to the conditions and limitations as specified therein.
If any parts, requirements, or ].imitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request must
be in the form of a written petition, conforming to Chapter 150E of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer .11666, Raleigh, North Carolina 27604. Unless such demands are made this
permit shall be final and binding.
One (1) set of approved plans and specifications is being forwarded to you. If

For the discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
construction and operation of a 1,000 GPD wastewater collection, treatment, and
disposal facility which consists of an existing 2,000 g411on capacity baffled septic
tank, an existing 420 square foot surface sand filter, am existing 73 gallon capacity
chlorination tank and tablet chlorinator, a 1,500 gallon pump tank with dual.
submersible 20 GPM pumps and high water alarm for pumping wastewater into the storage
pond, a minimum 30,000 gallon capacity storage pond, a pump station with a simplex 46
GPM submersible pump and high water alarm for pumping wastewater to the spray
irrigation field, a 46,200 square foot spray irrigation field and all piping, valves,
and appurtenances to serve the. Liselotte Rest Home Complex with no discharge to the
surface waters of the State, pursuant to the application received September 2, 1988,
and in conformity with the project plan, specifications, and other supporting data
subsequently filed and approved by the Department of Natural Resources and Community
Development and considered a part of this permit.
This permit shall be effective from the date of issuance until November 30, 1993,
and shall be subject to the following specified conditions and limitations;
1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications, and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable.
5. This permit shall become voidable in the event of failure of the soil to
adequately absorb the wastes and may be rescinded unless the facilities are
installed, maintained, and operated in a manner which will protect the assigned
water quality standards of the surface waters and ground waters.
6. In the event that the facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take such immediate
corrective action as may be required by this Division, including the construction
of additional or replacement wastewater treatment or disposal facilities.
7. The Raleigh Regional Office, phone no. 919/733-2314 shall be notified at least
twenty-four (24) hours in advance of operation of the installed facilities so
that an in-place inspection can be made. Such notification to the regional
supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00
p.m. on Monday through Friday, excluding State Holidays.
8. Any monitoring deemed necessary by the Division of Environmental Management to
insure surface and ground water protection will be established and an acceptable
sampling reporting schedule shall be followed.
9. The sludge generated from these treatment facilities must be disposed of in
accordance with General Statute 143-215.1 and in a manner approvable by the North
Carolina Division of Environmental Management.
10. A suitable year round vegetative cover shall be maintained.
11. Irrigation shall not be performed during inclement weather or when the ground is
in a condition that will cause runoff.
12. Adequate measures shall be taken to prevent wastewater runoff from the spray
field.
13. Diversion or bypassing of the untreated wastewater from the treatment facility
is prohibited.
14. The Permittee, at least six (6) months prior to the expiration of this permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the .facilities described therein, and if warranted, will
extend the Permit for such period of time and under such conditions and
limitations as it may deem appropriate.
15. The application rates shall not exceed 0.25 inches per acre per week.
16. The Permittee shall employ a certified wastewater treatment plant operator to be
in responsible charge of the wastewater treatment facilities. Such operator must
hold a certificate of the grade at least equivalent to the classification
assigned to the wastewater treatment facilities by the Certification Commission..
17. Freeboard in the lagoon shall not be less than two feet at any time.
18. Adequate facilities shall be provided to prevent surface runoff from carrying any
disposed or stored material into any surface waters.

19. The issuance of this permit shall not relieve the Permittee of the responsibility
for damages to surface or groundwaters resulting from the operation of this
facility.
20. The facilities shall be effectively maintained and operated as a non -discharge
system to prevent the discharge of any wastewater resulting from the operation of
this facility.
21. No type of wastewater other than that from the Liselotte Rest Home Complex shall
be sprayed onto the irrigation area.
22. No traffic or equipment shall be allowed on the disposal area except while
installation occurs or while normal maintenance is being performed.
23. The sand media of the surface sand filter must comply with the Division's
specifications. The engineer's certification will serve as proof of this
compliance.
24. Plans and specifications for sewer lines must be retained for five (5) years
after completion of construction, also plans and specifications for all other
projects must be retained for the life of the project.
25. A fence and warning signs shall be placed around the spray site.
26. Adequate measures shall be taken to stabilize the sprinkler nozzle risers and to
protect against freezing.
27. Upon final inspection and approval of these facilities by the Division, the
permit holder should request that the NPDES permit (N(30037290) be rescinded.
28. Adequate measures should be taken to ensure that operational interruptions, due
to equipment repair, are minimized.
29. The interior surfaces of the holding lagoon shall be completely lined with a
20 -mil thick UV resistant liner.
30. Following the installation and inspection of the lagoon liner and prior to waste
disposal operations, certification of the liner's compliance with the approved
construction specifications and the liner's integrity must be provided to the
Divisionn by the project engineer,
31. Upon completion of construction and prior to operation of the permitted facility,
a certification must be received from a professional engineer certifying that the
permitted facility has been installed in accordance with the requirements of this
permit and the approved plans and specifications. Mail the certification to the
Permits and Engineering Unit, P. 0, Box 27687, Raleigh, N. C. 27611-7687.
