PERMISSION IS HEREBY GRANTED TO

construction and operation of a wast.ewate.r. recycle :zyst(l!"I which consists of two
solids settling basins in series (40 €t-. by 1_t) ft.. by 6 ft.. and 60 ft. by 20 ft. by 6
ft.), a 96 GPM and a 50 GPM recirculation pump, a. 1.00 ft. flim« 10A, an 80 GPM pump
which is used to pump make-up water. .from the ahnn.doned mine, n 90 ft. by 105 ft. math
pond, and all appurtenances to serve the Blue Ri_dg(:� Gnmstone Hine anal Campground,
Inc. -tourist gem mining operation with r.io discharge t.o the slirFace waters of the
State, pursuant to the Application received July 25, 1988, nud in conformity with
the project plan, specifications, and other_ suppor.t.i37g clnt.n s3ibsequentl.y filed and
approved by the Department of Natural Resonrcas and Community Devcalopme"t and
considered a part of this permit.
This permit shall be effective from the date of issnnnc n. until September 30,
1993, and shall be subject to the following specified pond i tions nerd limitations:
I. This permit shall become voidable unless the faciliti.rs are constructed in
accordance with the approved plans, spec..i f icnt:ions rind ol:hnr supporting data.
2_ This permit is effective only with respect: Lo the 11,11111-F, and Volume of wnstes
described in the application and other supporting dnt.n.
3. The facilities shall be properly maintained and opornhod nt it 1 times.
4_ This permit is not transferable.
5. In the event that the facilities fail to perform sntisfnrtnrily, including the
creation of rmisance conditions, the Permittee shall Lnko such immediate
corrective action as may he required by this Division, including the construction
of Additional or replacement wastewater_ treatment or disposal facilities.
6. The Asheville Regional Office, phone no. 704/251-6208 shall he notified at
least twenty-four. (24) hours in advance of oparnI:kni of the installed
facilities so that an in-place inspection can he made. S,rch roti Fication to the
regional supervisor shall be made during the normal tiff ice hours from 8:00 n.m.
until 5:00 p.m. on Monday through Friday, excluding State Holidays.
7. Any monitoring deemed necessary by the Division of Environmental Management to
insure surface and ground water protection will lin nsLQ1 isbnd and an acceptable
sampling reporting schedule shall be followed.
8. The Permittee, at least six (6) months prior to Hin expirnl,i.on of this permit,
shall request its extension. Upon receipt of I.he requn.st:, the Commission will
review the adequacy of the facilities described th.n.r.n.in, and if warranted, wi.l.1
extend the Permit for such period of time and rrndor Such conditions and
limitations as it may deem appropriate.
9, The facilities shall be effectively maintained and opnro nd as a non -discharge
system to prevent the discharge of any wastewater resulting from the operation of
this facility.
10. Adequate inspection, maintenance, and cleaning shall hn provided by the Permittee
to insure proper operation of the subject facilities.
.11. A copy of the approved plans and specifications shA l l hr?. mn i,ntniTled on File: by
the Permittee for a minimum of five years from the data of iss"ance.
12. No wastewater from the mining operation shall be disrlinrgod intro the abandoned
mine shaft.
