In accordance with the provisions of Article 21 of Chapter_ 143, General SL.atutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

construction and operation of a 500 GPD subsurface westavat:nr t;reatmant and disposal.
system consisting of a 1500 gallon oil/water separator, two 1500 gallon storage/pump
tanks with a 64 gpm simplex submersible pump, and approximately 600 1-Inear feet: of
conventional nitrification trench for subsurface disposal., with all associated
piping, valves, and appurtenances to serve the Ryder Truck Rental, Inc. facility
located in Garner, NC, pursuant to the application received May 25, 1988, and in
conformity with the project plan, specifications, and Mhor supporting data
Subsequently filed and approved by the Department of NaWral Resonrces and Community
Development and considered a part of this permit.
This permit shall by effective from the date of i.sfivance until September 30,
1993, and shall be subject to the following specified conditions nncd 1 i.m.iLati.ons:
I. This permit shall become voidable unless the faci 1 Ai.on are consLruc>d in
accordance with the approved plans, specifications, and otbnr supporting cdaLa.
2. This permit: is effective only with respect" to L.lie. nAt ure and volume of wastes
described in the application and other supporting data.
3. The facilities shall be properly maintained and ope.ratRd K ,all times.
4. This permit is not transferable.
5. Upon the availability of a municipal or regional sewerage collection system, the
subject wastewater treatment facilities sbaJA he abandoned and ill. wast: ,water
discharged into the municipal or regional sewerage system.

6. This permit shall become voidable in the event of failure of Lhe soil to
adequately absorb the wastes and may be rescinded unless the facilities are
installed, maintained, and operated in a manner which will. protect the assigned
water quality standards of the surface waters and ground waters.
7. In the event that the facilities fail to perform satisfactorily, includi.ag the
creation of nuisance conditions, the Permittee shall take such immediate
corrective action as may be required by this Division, including the construction
of additional or replacement wastewater treatment or disposal Facilities.
8. The Raleigh Regional Office, phone no. 919/733-2314 shall be. notified at least
twenty-four (24) hours in advance of backfilling of' the installed non -discharge
treatment system so that an in-place inspection can be made. Such notification
to the regional supervisor shall be made during the normal office hours from 8:00
a.m. until. 5:00 p.m. on Monday through Friday, excludiug State Holidays.
9. The sludge generated from these treatment facilities must be disposed of in.
accordance with General Statute 143215.1 and in n manner approvable by the [forth
Carolina Division of Environmental. Management.
10. The Permittee, at least six (6) months prior to the expi-ration of: this permit.,
shall request its extension. Upon receipt of the regnnst, the Commission will
review the adegr.tacy of the facilities described therein., grad if warranted, will.
extend the Permit for such period of time and under such conditions and
limitations as it may deem appropriate.
.1.1.. The application rates shall not exceed 0.3 gallons/square Boot/day.
12. The .issuance of this permit shall not relieve the Permi.t.teo of the responsibility
for damages to surface or g.roundwater.s r.esulLi..ng From the operation of this
facility.
13. The facilities shall he effectively maintained and opc-..r.atc.'d as a non -discharge
system to prevent the discharge of any wastewater resulting from the operation of
this facility.
14. No traffic or equipment shall be allowed on the disposal area except while
installation occurs or while normal_ maintenance is being performed.
1.5. Adequate :inspection, maintenance, and cleaning shall by provided by the Permi.tLoe
to insure proper operation of the subject facilities.
16. In any future transfer of this land, a notice shall aregiven to the new owner
that gives full details of the materials applied or incor-porKed at, this site.
17. An acceptable reserve area, equal in size to the approved disposal area, shall be
maintained at all times.
1.8. Solids, sludges, or other pollutant—, removed or- rc+sult.dng from thn wastewater
storage facilities shall be contained and disposed of in such a manner as to
E prevent any contamination of the surface or groundwaters of the State.

19. A copy of the approved plans and specifications shall be maintained on file by
the Permittee for a minimum of five years from the date of issuance.
20. The groundwater supply well located approximately 250 linear feetnasL of the
proposed disposal area, must be abandoned according to reg"Inn ions in 15 NCAG 2C,
prior to operation of the wastewater disposal system.
21.. Oil drained from engine crankcases or degreasing solvents wastes sh.a.11 not be
discharged into the wastewater disposal system.
22. Any additional groundwater quality monitoring ns deemed necessary by the
Department, shall be provided.
23. Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a Professional engineer
certifying that the permitted facility has been installed in accordance with the
approved plans and specifications. Mail the certf[i.caLion to the Permits and
