Dear Mr. Williams:
In accordance with your application received August 26, 1988, we are
forwarding herewith Permit No. WQ0000250, dated February 28, 1989, to Lydall,
Inc, for the continued operation of the subject wastewater disposal facilities.
This permit shall be effective from the date of issuance until July 31, 1990,
shall void Permit No. 3622R2 and shall be subject to the conditions and limitations
as specified therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request must
be in the form of a written petition, conforming.to Chapter 150B of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer 11666, Raleigh, North Carolina 27604. Unless such demands are made this
permit shah be final and binding.

For the discharge of Sewage, Industrial Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

Yadkin County
FOR THE
continued operation of a 0.4 acre evaporation lagoon, a O.40 acre emergency holding
pond, and an in -plant transfer pump to serve Lydall, Inc. with no discharge to the
surface waters of the State, pursuant to the application received August 26, 19$"
and in conformity with the project plan, specifications, and other supporting da
subsequently filed and approved by the Department of Natural Resources and Community
Development and considered a part of this permit.
This permit shall be effective from the date of issuance until July 31, 1990,
shall void Permit No. 362282, and shall be subject to the following specified
conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications and other supporting data_
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable.
5. Adequate facilities shall be provided to prevent surface runoff from carrying
any disposed or stored material into the receiving stream.
6. In the event that the facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take such immediate
corrective action as may be required by this Division, including the construction
of additional or replacement wastewater treatment or disposal facilities.
7. Reports on the operations of the facilities shall be submitted to the
} Winston-Salem Regional Office at regular intervals and in such form and detail
as may be required by this Division.
8. The Permittee, at least six (6) months prior to the expiration of this permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the facilities destrribed therein, and if warranted, w-11
extend the Permit for such period of time and under such conditions and
limitations as it may deem appropriate.
9. The Permittee shall employ a certified wastewater treatment plant operator to be
in responsible charge of the wastewater treatment facilities. Such operator must
hold a certificate of the grade at least equivalent to the classification
assigned to the wastewater treatment facilities by the Certification Commission.
10. In the event that the liquid level of the pond approaches the two (2) feet
freeboard level, sufficient aeration will be provided to increase the
evaporation or the emergency holding pond shall be utilized.
11, The three (3) existing monitor wells shall be sampled every March, July and
November for the following parameters:

*(Analyses shall be by EPA Methods 601, 602, or 606 as appropriate).
The results of the sampling and analysis shall be sent to the N. C. Division
of Environmental Management of Form GW -59 (Compliance Monitoring Report Form)
every April, .August and December.
12. Any additional groundwater quality monitoring, as deemed necessary by the
Department, shall be provided.
13. The sludge generated from this facility must be disposed of in accordance with
requirements of 15 NCAC 2H and in a manner approvable by the N. C. Division of
Environmental Management.
Permit issued this the 28th day of February, 1989.
