Dear Mr. Reynolds:
In accordance with your application received August 26, 1988, we are
forwarding herewith Permit No. WQ0000236, dated September 29, 1988, to Mr. Pete
Reynolds for the construction and operation of the subject wnstewater spray
irrigation disposal system.
This permit shall he effective from the date of issuance. imiti.l September 30,
1993, and shall be subject to the conditimis and limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to .request an a.djudiratory hearing upon
written request within 30 days following receipt of this permit. This request must
be in the form of a written petition, conforming to Chapter 1508 of North Carolina
General. Statutes, and filed with the Office of Admin:i.strative. Hearings, Post Office
Drawer 11.666, Raleigh, North Carolina 27604. Unless Such demands are made this
permit shall be final and banding.

For the discharge of Sewage, Industrial Wastes, or Other. Wastes
In accordance with the provisions of Article 21 of Chapter 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
construction and operation of a 360 GPD single family spray irrigation system
consisting of a 1,000 gallon septic tank, a 1.,000 gallon pimp tank with submersible
pump, sand filter, tablet chlorinator, 1,000 gallon holding tank, 1,000 gallon pump
tank with submersible pump and high water alarms and a 8,712 sq. ft. spray irrigation
fieldto serve the Reynolds residence with no discharge to the surface wagers of the
State, pursuant to the application received August 26, 1988, and in conformity with
the project plan, specifications, and other support_i.ng data subsequently filed arid
approved by the Department of Natural Resources and Community Development and
considered a part of this permit.
This permit shall be effective from the date of issuance until September 30,
1993, and shall be subject to the following specified condi.ttions and lim.itations:
1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data..
3. The facilities shall be properly maintained and operated at: all times.
4. This permit is not transferable.
5. The Operational Agreement between the Permittee and the Environmental Management
Commission. is incorporated herein by reference and is a condition of this
Permit. Noncompliance with the terms of the Operational. Agreement shall subject
the Permittee to all sanctions provided by G. S. .1.43-2_1.5.6 for violation ' of or
failure to act in accordance with the terms and conditions of this Permit.
3 6. Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional. engineer
certifying that the permitted facility has been installed in accordance with the
requirements of this permit and the approved plans and spec.i_fications. flail the
certification to the Permits and Engineering Unit, P.O. Box 27687, Raleigh, N.C.
27611-7687.
7. A copy of the approved plans and specifications shall, be maintained on file by
the permittee for a minimum of five years from the date of issuance.
8. Upon the availability of a municipal or regional sewerage col.lecti.on system, the
subject wastewater treatment facility shall. be abandoned and all wastewater
discharged into the municipal or regional sewerage system.
9. This permit shall become voidable in the event of failure of the soil to
adequately absorb the wastes and may be rescinded unless the facilities are
installed, maintained, and operated in a manner which will protect the assigned
water quality standards of the surface waters and ground waters.
10. In the event that the facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take such immediate
corrective action as may be required by this Division, including the construction
of additional. or replacement wastewater treatment or. disposal. Facilities.
11. The Raleigh Regional. Office, phone no. 919/733-2314 shall be notified at least
twenty-four (24) hours in advance of operation of the installed system so that
an in-place inspection can be made. Such notification to the regional supervisor
shall be made during the normal office hours from 8:00 a.m. aintil_ 5:00 p.m. on
Monday through friday, excluding State Holidays.
12. Any monitoring deemed necessary by the Division of Environmental Management to
insure surface and ground water protection will be established and an. acceptable
sampling reporting schedule shall be ;Followed.
13. The sludge generated from these treatment facilities must: be disposed of in
accordance with General Statute 143-215.1 and in a manner. approvable by the North
Carolina Division of Environmental Management.
14. The spray irrigation system shall. be effectively maiiatainad and operated at all
times so that there is no discharge to the surface waters, nor any contamination
of ground waters which will render them unsatisfactory for normal. iise. In the
event that the facilities fail to perform satisfactorily, i.€icluding the creation
of nuisance conditions or failure of the irrigation urea to adequately absorb the
wastewater, the Permittee shall take such immediate. coi-ro.ci;ive action as may be
required by the Division of Environmental Management_
15. A suitable year round vegetative cover shall be mnfntai.n(,d.
16. Irrigation shall not be performed during inclement went.her or when the ground is
in a condition that will cause runoff.
1 17. Adequate measures shall be taken to
q prevent wastewater runoff :from the spray
field.

18. The Permittee, at least six (6) months prior to the expiration of this permit,
shall request its extension. Upon receipt of the request., the Commission will
review the adequacy of the facilities described therein, and if warranted, will
extend the Permit for such period of time and under such conditions and
limitations as it may deem appropriate.
19. The applic5tion rates shall not exceed:

20. The issuance of this permit shall not relieve the Permittee of: the responsibility
for damages to surface or g.roundwaters resulting from the operation of this
facility.
21. The facilities shall. be effectively maintained and operated as a non -discharge
system to prevent the discharge of any wastewater resulting from the operation of
this facility.
22. No type of wastewater other than that from Reynolds Rrsidonce shall. be sprayed
onto the irrigation area.
23. The Permittee is responsible for the operation and mni_intenance of the entire
treatment and disposal system including, but not limited to, the following :Mems:
a. The septic tank shall be checked every six (6) months and pumped out as
required.
b. The tablet chlorinator shall be checked every week until the actual usage rate
is established and thence on a monthly basis. Tablets shall_ be added so as to
provide proper chlorination.
c. The storage system, pump system, and alarm system s011 be inspected monthly.
The scum layer shall be removed at the same interval as the septic tank is
cleaned out.
d. The spray system shall. be inspected monthly to make certain of the sprayers
proper operation, that the, vegetative growth allows proper spray patterns,
that the soil is absorbing the disposed treated wastewater with 110 surface
runoff, and that no objectionable odors are being generated.
24. This permit may be voided if an operation and maintenance agreement is not signed
by the Permittee and a signed copy returned to this Division.
25. To the maximum extent feasible, the spray irrigation Finld, sand filter and
septic tank should not be located within 1.00 ft. of a potable drinking water
well. In no case shall this system be located within 50 ft, of a potable
drinking water well.
26. If excavation into bedrock for septic tank or said filter installation is
required, the pit shall be lined with a 10 mil synthetic liner.;'
