Dear Sirs:
In accordance with your request received August 10, 1988, we are forwarding
herewith Permit No. WQ0000132, dated October 13, 1988, to Mr. William A. Blake
and Mr. Kenneth A. Moore for the continued operation of the subject wastewater
collection, treatment, and disposal facilities.
This permit shall. be effective from the date of .issuance until September 30,
1993, shall replace Permit No. 9330R, and shall be subject to the conditions and
limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request art adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request must
be in the form of a written petition, conforming to Chapter. 150B of North Carolina.
General Statutes, and filed with the Office of Administ:ra.ti:ve Hearings, Post Office
Drawer_ 11666, Raleigh, North Carolina 27604. Unless such demands are made this
permit shall be final and binding.

In accordance with the provisions of Article 21 of Chapter- 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

continued operation of a 450 GPD spray irrignt!on wastewater disposal facility on
the Kenneth A. Moore property consisting of a 1,000 gallon septic tank, dual
subsurface sand filters, disinfection, two (2) 1,500 gallon pump/storage tanks with
disposal to a. 0.35 acre fenced spray field and the continued. operation of all
identical 450 GPD wastewater spray irrigation (nullity located on the property of
William A. Blake and all related piping and appur.t:ennnces to serve both the William
A. Blake residence and the Kenneth A. Moore .residence, pursuant to the request
received August 10, 1988, and in conformity with the project: plan., specifications,
and other supporting data subsequently filed and approved by the Department of
Natural Resources and Community Development and considered a part. of this permit.
This permit shall be effective from the date of issuance until September 30,
1993,.shall replace Permit No. 9330R, and shall be subject: to the following specified
conditions and limitations:
1.. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications and other supporting data.
2. This permit is effective only with respect to t.tre nat.urc,. and volume of wastes
described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable.
5. Upon the availability of a municipal or regional sewerage collection system, the
subject wastewater treatment facilities shall be abandoned and a].l wastewater
discharged into the municipal or regional sewerage system.
6. This permit shall become voidable in the event: of failnr_e of the soil to
adequately absorb the wastes and may be rescinded unless the facilities are
installed, maintained, and operated in a manner which will protect the assigned
water quality standards of the surface waters and ground wagers.
7. In the event that the facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take such immediate
corrective action as may be required by this Division, including the construction
of additional or replacement wastewater treatment or disposal facilities.
8. Any monitoring deemed necessary by the Division of Environmental Management to
insure surface and ground water protection will be established and an acceptable
sampling reporting schedule shall be followed.
9. The sludge generated from these treatment facilities must be disposed of in
accordance with General Statute 143-215.1 and in a manner approvable by the North
Carolina Division of Environmental Management.
10. A suitable year round vegetative cover shall be maintained.
11. Irrigation shall not be performed during inclement weather or when the ground is
in a condition that will cause runoff.
12. Adequate measures shall be taken to prevent wastewater runoff from the spray
field.
13. Diversion or bypassing of the untreated wastewater from the treatment facility
is prohibited.
14. The Permittee, at least six (6) months prior- to the, expiration of this permit,
shall request its extension. Upon receipL of the request, the Commission will,
review the adequacy of the facilities described therein, and if warranted, will
extend the Permit for such period of time. and under such conditions and
limitations as it may deem appropriate.
15. The application rates shall not exceed:
a. 1.25 inches per week
b. 0.5 inches per hour
16. Adequate facilities shall be provided to prevent surfacee runoff from carrying any
disposed or stored material into any surface waters.
17. The issuance of this permit shall not relieve the Permittee of the responsibility
for damages to surface or groundwaters resulting from the operation of this
facility.
18. The facilities shall be effectively maintained and operated as a non -discharge
system to prevent the discharge of any wastewater resulting from the operation of
this facility.
19. No type of wastewater other than that from the Blake and Moore Residences shall
be sprayed onto the irrigation area.
20. No traffic or equipment shall be allowed on the disposal area except while
installation occurs or while normal maintenance is being performed.
21. Adequate inspection, maintenance, and cleaning shall. be provided by the Permittee
to insure proper operation of the subject facilities.
22. The Permittee is responsible for the operation and maintenance of the entire
treatment and disposal system including, but not limited to, the following items:
a. The septic tank shall be checked every six (6) months and pumped out as
required.
b. The tablet chlorinator shall be checked every week until the actual usage rate
is established and thence on a monthly basis. Tablets shnil be added so as to
provide proper chlorination.
c. The storage system, pump system, and alarm system shall be.inspected monthly.
The scum .layer shall be removed at the same interval as the septic tank is
cleaned out.
d. The spray system shall be inspected monthly to make certain of the sprayers
proper operation, that the vegetative growth allows proper spray patterns,
that the soil is absorbing the disposed treated wastewater with no surface
runoff, and that no objectionable odors are being generated.
23. This permit may be voided if an operation and maintenance agreement is not signed
by the Permittee and a signed copy returned to this Division.
24. Wastewater shall not be applied daring inclement. weather- or until twenty-four
(24) hours following a rainfall event of. 0.5 inches or greater in twenty-four
(24) hours or when the ground is frozen or in a cond:iLion which would result in
surface runoff.
25. This permit shall become voidable unless the agreement between Mr. Jewell R.
Keith and Mr. and Mrs. William A. Blake for the easements that provide area for
the subject spray field on Mr. Keith's property Is i.n full force and effect:.
26. Adequate buffer zones shall be main.tainf-d as follows:
a. 100 -ft. to property lines and easement limits
b. 400 -ft. to residences on adjoining property
c. 200 -ft. from the Moore Residence
d. 200 -ft. from any water course
Permit issued this the 13th day of October., 1988.
