Dear Mr. Brown:
In accordance with your application received July 25, 1988, we are forwarding
herewith Permit No. WQ0000026, dated October 11, 1988, Lo Clark Stone Company for
the construction and operation of the subject wastewater recycle system.
This permit shall. be effective from the date of issuance. anti]. October 31, 1993,
and shall be subject to the conditions and limitations as specified therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of, this permit:. This request must
be in the form of a written petition, conforming to Chapter 150B of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer 11666, Raleigh, North Carolina 2760+. Unless stitch demands are made this
permit shall be final and binding.

For the discharge of Sewage, Industrial. Wastes, or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General Statutes
of North Carolina as amended, and other applicable haws, Rules, and Regulations
PERMISSION IS HEREDY GRANTED TO

FOR THE
construction and operation of a 4,800 GPD wastewater recycle system consisting of a
105' x 96' x 15' supply pond, a 200 GPM supply pump, n screening device, two settlin;
ponds 1.00' x 1.80' x 15' and 120' x 130' x 15' connenW.d in series, a 100 GPI•
recirculation pump and all other piping and appurtances. to servo Clark Stone Company
with no discharge to the surface waters of the stat:(,, pursuant to the application
received July 25, 1988, and in conformity with the proinct plan, specifications, and
other supporting data subsequently filed and approved by t:l?E, Depa.rtmFnt. of Na Lural
Resources and Community DeveI.opment. and considered a part of this permit.
This permit shall be effective from the late of :issuance until October 31., 1993,
and shall be subject to the following specified conditions anti limitations:
1. This permit: shall become voidable unless the Wil it:i.es are constructed in
accordance with the approved plans, specifications acrd othar supporting data.
2. This permit is effective only with respect to t:hn nstn rn and volume of wastes
described in the application and other supporting rlaLn.
3. The facilities shall be properly maintained and oporaLod nu nil times.
4. This permit is not transferable.
5. Construction of the sewers shall be so scheduled so as Tref: to interrupt service
by the existing utilities nor result in an overflow or hypass discharge of
wastewater_ to the surface waters of the State.
6. A copy of the approved plans and specifications shall be maintained on file by
the permittee for a minimum of five years from the date of issuance.
7. In the event that the facilities fail to perform satisfactorily, including the
creation of nuisance conditions, the Permittee shall take such immediate
corrective action as may be required by this Division, including the construction
of additional or replacement wastewater treatment or disposal facilities.
8. Any monitoring deemed necessary by the Division of Environmental Management to
insure surface and ground water protection will be established and an acceptable
sampling reporting schedule shall be followed.
9. The Permittee, at least six (6) months prior to the expiration of this permit,
shall request its extension- Upon receipt of the request, the Commission will
review the adequacy of the facilities described therein, and if warranted, will
extend the Permit, for such period of time and undar such conditions and
limitations as it may deem appropriate.
10. Adequate facilities shall be provided to prevent surface runoff from carrying any
disposed or stored material into any surface waters.
11. The facilities shall be effectively maintained fuml operated a.s a non -discharge
system to prevent the discharge of any wastewater resulhing from the operation of
this facility.
12. The sludge generated from "these tr.eatmeut- facKiLios must be disposed of in
accordance with General Statute I43-215. % and in a mann(. r- approvable by the
North Carolina Division of Environmental Management..
